Job Overview
Ina Grafton Gage Home is seeking a Registered Nurse (RN) to fill an existing temporary vacancy covering an approved leave of absence for approximately 14 months. Reporting to the Assistant Director of Care/Nurse Manager, the RN provides professional nursing care, clinical leadership, and oversight to residents while working collaboratively with the interdisciplinary team to deliver safe, person-centred care. During night shifts, the RN is responsible for the clinical oversight of two home areas and serves as the building's clinical lead.
Shift: Night 11PM to 7AM, 10 Shifts every two weeks with rotating weekends.
Responsibilities
- Provide safe, compassionate, and person-centred nursing care that promotes resident dignity, independence, choice, and quality of life.
- Perform comprehensive nursing assessments, identify changes in resident condition, and develop, implement, and evaluate individualized plans of care.
- Administer medications, treatments, and nursing interventions safely in accordance with physician orders, CNO standards, and the Home's policies.
- Maintain accurate, timely, and complete clinical documentation, including assessments, progress notes, care plans, medication records, and required reporting.
- Collaborate with physicians, nurse practitioners, residents, families/substitute decision-makers, and the interdisciplinary team to coordinate high-quality resident care.
- Provide clinical leadership, direction, and support to Registered Practical Nurses, Personal Support Workers, and other members of the care team.
- Promote best practices in infection prevention and control (IPAC), medication safety, resident safety, and quality improvement.
- Participate in resident admissions, transfers, discharges, care conferences, interRAI assessments, and other clinical care planning activities.
- Recognize and respond appropriately to changes in resident condition, urgent situations, and medical emergencies, ensuring timely communication and follow-up.
- Ensure compliance with the Fixing Long-Term Care Act, 2021, Ontario Regulation 246/22, College of Nurses of Ontario standards, PHIPA, Occupational Health and Safety legislation, and the Home's policies and procedures.
- Support a positive resident and family experience through professional communication, advocacy, and timely resolution of concerns.
- Contribute to quality improvement initiatives, inspections, audits, and continuous improvement activities to support excellence in resident care.
- During night shifts, provide clinical oversight for two home areas and act as the senior clinical lead for the Home, supporting staff, responding to clinical concerns, and coordinating after-hours operations.
- Perform other duties as assigned.
Qualifications
- Current registration in good standing with the College of Nurses of Ontario (CNO) as a Registered Nurse (RN).
- Current Basic Life Support (BLS)/CPR certification.
- Previous experience in long-term care, gerontology, or a related healthcare setting is considered an asset.
- Demonstrated knowledge of the Fixing Long-Term Care Act, 2021, Ontario Regulation 246/22, CNO Professional Standards, and nursing best practices.
- Working knowledge of interRAI assessments, care planning, and electronic clinical documentation is considered an asset.
- Strong clinical assessment, critical thinking, problem-solving, and decision-making skills.
- Demonstrated leadership abilities with the capacity to provide clinical direction, mentor team members, and foster effective interdisciplinary collaboration.
- Ability to work independently, prioritize competing demands, and respond confidently and effectively in urgent and emergency situations.
- Satisfactory Vulnerable Sector Check (including a Criminal Record Check) completed within the last six (6) months, or willingness to obtain one as a condition of employment.
- Documentation of tuberculosis (TB) screening in accordance with Public Health requirements and the Home's policies as a condition of employment.
- Proof of immunization and vaccination status as required by the Home.

Pay: $39.07-$56.00 per hour
