This position will enforce safety standards, coordinate multidisciplinary admissions, transfers, and discharges, and ensure accurate clinical documentation.

The Responsibilities
- Work with management and the interdisciplinary team members to provide RAI-MDS education to interdisciplinary team members as required.
- Ensure the scheduling and completion of the RAI-MDS assessment, Resident Assessment Protocols (RAPs) and care plan by the interdisciplinary team members on a quarterly basis.
- Oversee the coordination of the RAI process including training, RAI communication, scheduling, data accuracy and data submission.
- Schedule support and consulting calls and evaluations with the Corporate RAI Consultant as required for review of assessment data and QI results.
- Attend scheduled consulting calls with required assessment data and information completed. Complete suggested revisions and follow ups as per RAI Consultant direction.
- Ensure a resident assessment schedule is developed, completed, current and accessible to all RAI users.
- Oversee the RAI process according to home policies, MOLTC regulations, College Standards of Practice and interRAI requirements (see CIHI User’s Manual).
- Ensure MDS assessments, resident assessment protocols and care planning are completed as scheduled and required by Omni Policy, interRAI and CIHI requirements.
- Ensure data submission is completed as required in accordance with Omni policy, interRAI and CIHI requirements, with approval from Corporate RAI Consultant.
- Provide RAI-MDS 2.0 education and computer application training to all RAI users.
Education
- A Bachelor of Science degree in Nursing OR a Diploma in Nursing.
- Certificate of competence as a Registered Nurse or Registered Practical Nurse with the College of Nurses of Ontario.
- At least one year of experience working as a registered nurse or registered practical nurse in the long term care sector.
- Successful completion of RAI-MDS Credentialing program or equivalent.
- A minimum of three years clinical, project leadership and management experience.
