Description
Position Summary
The Registered Dietitian provides high-quality, person-centered nutrition services across primary care, hospital and long-term care settings. This includes nutrition assessment, care planning, intervention, education, and documentation for patients and residents with diverse medical, surgical, chronic disease, and geriatric needs. The Registered Dietitian works collaboratively within interdisciplinary teams to support nutritional health, disease management, recovery, and quality of life
Position Relationships
Reports to the Manager of Clinical Services – Primary Care and communicates and interacts with the healthcare team including physicians, other staff and managers, students and volunteers.
Duties:
- Conduct comprehensive nutrition assessments and develop individualized nutrition care plans for patients and residents in primary care, acute care, and long-term care settings.
- Provide evidence-based nutrition counselling, education, and therapeutic dietary recommendations to support health promotion, disease prevention, and the management of acute and chronic health conditions.
- Collaborate with physicians, nurses, and other interdisciplinary team members to support care planning, treatment goals, and safe care transitions.
- Educate patients, residents, families, caregivers, and staff through individual consultations, group education sessions, and the development of nutrition resources.
- Maintain accurate clinical documentation and ensure compliance with professional standards, organizational policies, and regulatory requirements.
- Participate in quality improvement initiatives, program development, and the evaluation of nutrition services to enhance patient and resident outcomes.
- Promote a culture of safety by adhering to infection prevention and control practices and supporting high-quality, patient- and resident-centered care.
- Adheres to all policies and procedures.
- Supports and demonstrates the mission, vision and values of the organization.
- Other duties may be assigned and may not be limited to the scope of an individual department.
Qualifications:
- Current registration in good standing with the College of Dietitians of Ontario required.
- A bachelor’s degree in foods, nutrition, or dietetics.
- Basic Rescuer (CPR) certification.
- Certified Diabetes Educator designation preferred.
- Member of Dietitians of Canada preferred.
- Experience in hospital, primary care, and/or long-term care settings is an asset.
- Knowledge of geriatric care principles and interRAI-LTCF or interRAI tools preferred.
- Proficient in electronic documentation and clinical systems.
- Ability to assess and treat clients across a range of ages, diagnoses, and care needs.
- Ability to build strong relationships with a variety of individuals including patients, residents, families, employees, and community partners.
- Initiative, sound judgment and attention to detail.
- Ability to work independently and be self-directed in a fast paced and dynamic environment.
- Maintain staff & patient/resident confidentiality.
- Demonstrated excellent time management, organization, communication and interpersonal skills required.
- Exceptional attendance required.
- Must agree to adhere to DRDH standards of behaviour.
