Job Overview:
The Family Intake Facilitator works within Terra’s Values and Practice Model, fostering relationships, community, and positive outcomes for young parents and their children. As the first point of contact for pregnant and parenting teens, young parents, and families, the Family Intake Facilitator provides a welcoming, trauma-informed, strengths-based intake experience across Terra’s programs and locations.
This role serves as a central access point for participants, helping families identify programs, funding opportunities, and community resources that align with their goals and needs. Responsibilities include coordinating intake processes, conducting screenings and assessments, facilitating program matching, managing referrals and waitlists, and supporting service navigation to ensure timely and equitable access to services.
The Family Intake Facilitator builds trusting relationships with participants, collaborates with internal teams and community partners, and advocates for family needs. Through responsive, inclusive, and participant-centered practices, the role supports engagement, resilience, and positive outcomes for young parents, children, and families.
Job Conditions:
This is a full time (35 hours/week) with flexible hours Monday through Friday, 2 evenings/week, and the occasional weekend based on the needs of families and staff and to support day/evening programming. The office space is split between Terra Centre at 10320 146 Street and Braemar School 9359 67a Street.
Key responsibilities:
- Serve as a positive ambassador for Terra Centre for Teen Parents by practicing in alignment with the organization’s mission, vision, values, policies, strategic goals, and practice model.
- Contribute to a collaborative team environment and participate in performance reviews, supervision, training, and organizational initiatives.
- Serve as the first point of contact for new and prospective participants, providing timely, welcoming, and trauma-informed intake, assessment, and service navigation support.
- Conduct comprehensive assessments to identify participant and family strengths, needs, goals, and interests, and connect them with appropriate programs, services, and community resources.
- Coordinate participant intake, registration, consent processes, referrals, and service transitions to ensure seamless access to supports.
- Facilitate applications and eligibility processes for programs and financial supports, ensuring accurate and complete documentation.
- Build trusting relationships with young parents and families, providing advocacy, guidance, and support to help them achieve personal and family goals.
- Empower participants to strengthen self-advocacy skills and independently access community resources, services, and natural support networks.
- Collaborate with families, internal teams, schools, community agencies, and service providers to coordinate responsive, participant-centered service delivery.
- Maintain accurate, confidential, and up-to-date participant records, case notes, reports, and database documentation in compliance with agency standards and privacy requirements.
- Monitor participant progress, collect outcome data, complete file audits, and contribute to program evaluation and reporting activities.
- Develop and maintain strong community partnerships and referral networks to enhance service access and support positive outcomes for participants and families.
- Represent the organization professionally within the community, promoting its mission, values, programs, and services.
- Participate in team collaboration, professional development, and continuous improvement initiatives while adhering to agency policies, ethical standards, and inclusive service practices.
Qualifications/Requirements:
- Post-Secondary Diploma or Degree in Social Work, or equivalent. A combination of comparable and relevant Human Services education, practicums, and work experience may be considered.
- Mandatory registration with corresponding applicable professional governing body.
- Demonstrated ability to think critically, problem solve and de-escalate situations in a fast-paced environment that requires the prioritization of tasks.
- The ability to work independently in a self-directed environment with strong time management skills is required
- Ability to develop positive, effective, professional rapport and relationships with youth.
- Knowledge of issues associated with people struggling with mental illness and substance abuse.
- Strong ability to advocate on behalf of program participants
- An understanding of the complex barriers for young families that requires strong advocacy efforts and partnership development to meet their needs within the community.
- Knowledge of referral services and supports within the Edmonton community and/or the ability to research and connect with community resources
- Understanding and knowledge of the issues young people face when pregnant and parenting
- Experience working with low income and marginalized participants, including people facing multiple barriers.
- Ability to work with families from diverse cultural backgrounds.
Knowledge and Abilities:
- Ability to work with families from diverse cultural backgrounds and develop positive, effective, professional rapport and relationships with youth.
- Knowledge of issues associated with people impacted by mental illness and problematic substance use.
- Knowledge of referral services and supports within the Edmonton community and/or the ability to research and connect with community resources.
- Knowledge of child/youth development and child-parent attachment.
- Understanding and knowledge of the issues pregnant and parenting young people face.
Other requirements:
- Valid driver’s license and a reliable vehicle available for regular use with 2 million liability insurance.
- Clear (or Executive Director approved) 3-year driver’s abstract.
- Ability to work flexible hours including two evenings per week.
- Manual dexterity required to operate various appliances and equipment.
- Ability to lift between 25- 50 lbs.
- Current Standard First Aid and CPR (Level C Intermediate).
- Applied Suicide Intervention Skills Training (ASIST).
- Clear (or Executive Director approved) Police Information Check (including vulnerable Sector search) completed 90 days prior to start date if selected as successful candidate.
- Clear (or Executive Director Approved) Intervention Record Check completed 90 days prior to start date if selected as successful candidate.
- Documentation of registration as a Social Worker as per provincial legislation if applicable.

Compensation: The starting salary for the position is $47,857.
Upon completion of a probationary period, we offer a comprehensive, competitive, extended health benefits package including RRSP contribution. Staff have three weeks of vacation, and, in addition, the agency is closed between Christmas day and New Years.
