Position Details:
- Position Title: Pastoral Care Coordinator
- Position Type: Part -Time
- Location: Chinook Care Center – Hospice Unit
- Status: Non- Union
Position Summary
The Pastoral Care Coordinator brings to the interdisciplinary team their knowledge and expertise in the provision of holistic care to palliative patients. They will participate in team meetings to manage complex patient care, family questions and concerns and address care planning needs. They will coordinate with the Social Worker to carry out a Bereavement program. They will assess and be sensitive to the patient and family’s existential needs in collaboration with the interdisciplinary team and offer spiritual support and resources. They will also assist and support the well-being of interdisciplinary team members when needed through spiritual debriefing and discussions.

Qualifications:
- Graduated from a recognized Chaplaincy Program.
- Registered with the Canadian Association for Pastoral Practice & Education (CAPPE).
- A minimum of two years’ experience in Pastoral Care, preferably within a palliative care environment.
- Ability to work autonomously within an interdisciplinary team environment.
- Demonstrates concern and interest in providing quality care and service for palliative residents.
- Mentally and physically able to perform duties as outlined.
- Ability to function as an effective team member with other staff, residents, families, and the public.
- Ability to read, write and comprehend the English language.
Key Responsibilities:
- Support patients and families by facilitating communication, providing guidance, and connecting them with resources.
- Assist patients and families in coping with illness, disability, or end-of-life challenges.
- Collaborate with colleagues to maintain a positive, effective team environment, offering constructive feedback and mentorship.
- Participate in meetings, committees, training, and organizational events to improve services.
- Maintain professional standards, follow policies, and pursue ongoing learning to enhance knowledge and skills.
- Promote workplace safety, adhere to health and fire protocols, and report hazards as needed.
- Perform other duties as assigned.
