PSW Program Scheduler

Reporting to the Manager, Client Contact Centre, the PSW Program Scheduler supports the day-to-day provision of services; vacations, gaps, additional hours of service and sick calls; coordinates the provision of In-Home Support and participates as an effective member of the Community Care team.

Responsibilities include but not limited to:

Scheduling Coordination

Oversees shift cancellations and address last minute scheduling changes by reassigning vacant visits;
Schedules PSWs and Homemakers to vacant client visits for the continuity of client care;
Completes visit confirmation calls with clients;
Confirms scheduled visits with PSWs and Homemakers and notifies them of any last-minute changes to their schedules;
Reviews care plans with PSWs to complete client visits in accordance with their care requirements;
Monitors PSW and Homemaker hours so staff are scheduled according to their contracted hours;
Requests supervisor for overtime in exceptional circumstances when additional coverage is required;
Responds to day-to-day inquiries from PSWs and Homemakers;
Communicates schedule updates, client information, and service changes to staff in a timely manner;

Client Services and Coordination

Receives and processes referrals and schedules the initial client visit;
Follows up on “Not Seen / Not Found” clients by confirming their whereabouts and initiating the wellness check process when required;
Supports PSWs and Homemakers and submitting incident reports with proper documentation;
Communicates with internal departments to coordinate client care and service delivery;
Handles sensitive phone calls with clients and staff, de-escalating concerns and escalating issues to the appropriate supervisor when necessary;

Documentation and Administrative Support

Documents all calls and interactions in the scheduling software to track attendance patterns, client health status updates, and operational issues;
Completes book-off forms to track staffing levels and statutory holiday hours for submission to payroll;
Maintains accurate records related to staffing schedules, client visits, and service delivery.

Qualifications required:

Post secondary education in Social Service Work, including Office Administration or Medical Administration, or a combination of education and experience is required
One (1) year of scheduling experience in a health care setting required
Strong interpersonal, communication and organizational skills
Proficient in Microsoft Office software (Word, Outlook, Excel, Teams, etc.)
Experience using a Customer Relationship Management (CRM) system is an asset
Ability to work in a fast-paced environment
Strong interpersonal, communication and organizational skills
Good working knowledge of the principles of community support services including community, mental health and gerontology, volunteer management
Must work every other weekend on rotation
Ability to work any hours within a 24-hour time period
Sensitivity to the needs of the vulnerable population with disabilities, including challenges in cognition  
Clear Police Check completed within the past 6 months
Must work in a safe manner and promote the health and safety of all staff, volunteers, clients and the community as outlined within CCD policies

Compensation: Hourly base rate of $22.00; with an additional 5% in lieu of benefits, and 6% vacation pay

Work Location: Pickering, ON

*Subject to change, CCD staff may be required to work anywhere throughout the Durham Region

Hours of Work: Casual (up to 35/hours per week). Open availability is required for shifts scheduled on weekdays and weekends, including mornings, afternoons, and evenings.
