Full job description

Overview

We are seeking an Intensive Case Manager (ICM) to provide intensive, ongoing support to participants experiencing complex behavioural, mental health, substance use, cognitive, emotional regulation, interpersonal challenges and social complexities.

The ICM works with participants in higher needs supported housing who may require long-term support, recognizing that progress may be non-linear and focused on building and sustaining safety, stability, quality of life and engagement with services. The ICM advances the mission of the Atlantic Community Shelters Society of finding each participant a “home for good”.

Responsibilities include:

Develop intensive individualized service plans.
Provide ongoing crisis intervention and behavioural support.
Conduct suicide risk, overdose risk, and safety assessments.
Utilize harm reduction and trauma-informed practices.
Develop behavioural support and safety plans.
Provide frequent one-on-one support for participants experiencing mental health crises or substance use and trauma.
Coordinate services with mental health, addictions, healthcare, corrections, probation, hospitals, and outreach providers.
Facilitate multidisciplinary case conferences.
Respond to behavioural incidents using approved de-escalation techniques.
Assist participants with permanent housing when appropriate and support transitions between supported housing locations.
Support participants with housing applications, landlord engagement, income assistance, and required documentation.
Complete referrals to By-Name Data and support Coordinated Access.
Maintain up to date participant records and case notes in internal database and external systems.
Maintain accurate, detailed, and timely documentation of participant interactions, case plans, and outcomes.
Provide regular updates to the Manager, Community Connections and Site Supervisor regarding participant progress and emerging risks.
Participate in case conferencing, coordinated access meetings, and team debriefings.
Collaborate with Site Supervisors and Shelter Support staff to address complex participant situations.
Engage in ongoing professional development and required training.
Support data collection and reporting requirements as directed.

Qualifications

Bachelor’s degree in social work, psychology, human services, nursing, or related discipline required.
Minimum 3–5 years of experience supporting individuals with complex needs preferred.
Experience in shelters, addictions, mental health, outreach, supportive housing, corrections, or healthcare required.
Strong knowledge of concurrent disorders and complex trauma.

Knowledge & Skills

Strong understanding of homelessness, trauma, harm reduction, mental health, and substance use.
Knowledge of systemic discrimination and barriers impacting equity-deserving populations in Nova Scotia.
Demonstrated de-escalation, behaviour management and conflict resolution skills.
Strong personal resilience and ability to maintain professional boundaries.
Strong organizational skills and ability to manage multiple participants effectively.
Excellent verbal and written communication skills.
Ability to work independently while contributing positively to a team environment.
Strong knowledge of community resources and housing systems.
Criminal Record Check approved by Manager of Human Resources.

Certifications

First Aid/CPR, Naloxone, NVCI, MANDT, Mental Health First Aid, and ASIST are required and training will be provided.

Pay: $28.00 per hour
