Care Coordinator

Temporary Full-Time – 6-months

At Community Care Durham (CCD), coordinated networks of volunteers and inter-professional staff, working out of integrated community hubs and other locations, strive to enrich the well-being and quality of life for people wherever they choose to call home. As the largest community support services agency in Durham Region, CCD collaborates with its partners and the broader community to address community issues and support local citizens to achieve their goals to live independently.

Reporting to the Program Manager, the Care Coordinator is responsible for assessing client and care partners’ needs in the community, while appropriately linking and referring clients to suitable resources through care navigation. The Care Coordinator provides ongoing case management to clients and care partners until discharge.

Responsibilities include but not limited to:

Provides exceptional customer service and support to an assigned case load - acting as a single point of contact for any CCD service;
Completes client assessments & re-assessments in the client's home (Ongoing assessment, planning and evaluation);
Co-facilitates intake and case management meetings, care/service planning and discharge meetings;
Facilitates problem solving and planning, and advocates for clients and their care partners by working directly with them, connecting them to community resources, and making appropriate referrals to internal or external community services providers;
Serves as a member of multidisciplinary team of professionals working with clients;
Provides interdisciplinary field support as required;
Updates client centered care plans through the identification of short- and long-term client’s goals;
Enters and updates client’s information, case notes and assessments into database;
Records all activities and develops reports for statistical purposes;
Attends agency staff meetings and participates in agency awareness initiatives and community events;
Performs other duties as required.

Qualifications required:

Post-secondary education in health/social services field
Minimum two (2) years of experience in health/social services field
RAI CHA Certification is an asset
Strong assessments skills both in person and over the phone, with the ability to identify needs, risks and urgencies with high-risk vulnerable clients
Valid “G” or “G2” driver’s license and access to reliable vehicle, with willingness to travel throughout the Durham Region
Valid First Aid and CPR certificate
Proficient computer skills (MS Office Suite)
Working knowledge of AlayaCare database is an asset
Strong verbal and written communication skills
Ability to work well under pressure to meet deadlines through flexibility and multitasking
Flexibility to work days, evenings and weekends as required
Sound knowledge of community Support Services, Care coordination and Customer service
Ability to effectively complete client assessment using a variety of different Health Information Management Systems an asset
Understanding of client communication portals and platforms
Strong critical thinking and creative problem-solving skills to identify and eliminate barriers and risks
Sensitivity to the needs of the vulnerable population with disabilities, including challenges in cognition  
Clear Vulnerable Sector Police Check completed within the past 6 months. (Please note: 4–6 weeks processing time)
Must work in a safe manner and promote the health and safety of all staff, volunteers, clients and the community as outlined within CCD policies

Compensation: Hourly base rate of $25.00; with an additional 5% in lieu of benefits, and 6% vacation pay

Work Location: Whitby, ON and throughout Durham Region

Hours of Work: 8:30am – 4:30pm (35 hours/per week). Alternating weekends
