Full job description

Join a collaborative, well-supported primary care team where you’ll play a key role supporting a physician’s patient panel and delivering high-quality, community-based care.

Ajax Family Practice & Walk-In Clinic is seeking a Nurse Practitioner to work closely with one of our family physicians in a structured, physician-led care model.

The Role

This position focuses on supporting a dedicated family physician by providing care to their rostered patients. You’ll play an important role in follow-ups, chronic disease management, and same-day access, while maintaining strong continuity of care under the physician’s direction.

Responsibilities

Conduct comprehensive patient assessments, diagnoses, treatment planning, and health education within the RN(EC) scope of practice and in accordance with CNO standards
Support the ongoing care of patients from an established physician’s roster, helping maintain continuity and strong long-term patient relationships
Manage routine primary care visits, follow-ups, preventative care, and chronic disease management for patients across the lifespan
Support same-day and urgent patient needs, including episodic care and physician coverage as needed
Order, interpret, and follow up on diagnostic tests, screenings, and investigations within scope
Provide patient counseling and education on treatment plans, medications, preventative care, lifestyle modifications, and community resources
Deliver compassionate, patient-centered, and culturally responsive care tailored to each individual’s needs and circumstances
Collaborate closely with the physician and clinic team to coordinate care, ensure appropriate follow-up, and support high-quality patient outcomes
Maintain accurate and timely clinical documentation within the OSCAR EMR system
Contribute to a positive, supportive, and team-oriented clinic environment focused on excellent patient care
Assist with preventative care initiatives, patient outreach, and chronic disease monitoring to help improve overall patient health outcomes

A commitment to continuity of care and fostering long-term patient relationships
Adaptability and professionalism in a dynamic primary care setting

About the Clinic

Physician-owned practice with multiple family physicians
High patient volume with an established patient base
Family practice + walk-in model
OSCAR EMR
Full administrative support
Two-floor clinic connected to an on-site pharmacy
Located in Ajax, Ontario

Required

Active practice registration with the College of Nurses of Ontario
Bachelor’s in Nursing; Master's Degree in Nurse Practitioner
Successful completion of the Ontario Primary HealthCare Nurse Practitioner Certificate Program is preferred
Current Membership in the Nurse Practitioners Association of Ontario and the Registered Nurses Association of Ontario (RNAO)
Advanced nursing care knowledge related to health assessment, diagnostic, pharmacotherapeutics and invasive and minor surgical procedures required
Minimum of one (1) year recent primary care nursing experience required
Excellent patient focus and communication skills
Experience with EMR is an asset
