Part-Time Care Coordinator – Durham Region
At The Core Home Health Inc. is seeking a professional, organized, and community-focused Part-Time Care Coordinator to support our growing operations throughout Durham Region.
This position combines healthcare marketing, community outreach, client coordination, scheduling support, and every-other-weekend on-call responsibilities. The successful candidate must be comfortable working independently, building professional relationships, supporting clients and caregivers, and representing At The Core Home Health within the community.
Position Details
- Part-time position
- Two community outreach and road days per week
- One in-office planning and coordination day per week
- Every-other-weekend on-call rotation
- Durham Region, Ontario
- Pay: $23.00–$26.00 per hour, based on qualifications and experience
Additional compensation related to the on-call rotation and position-related travel will be discussed during the interview process.
Key ResponsibilitiesCommunity Outreach and Healthcare Marketing
The Care Coordinator will spend two days per week travelling throughout Durham Region to:
- Visit hospitals, medical clinics, retirement residences, pharmacies, community agencies, seniors’ organizations, and other potential referral partners
- Introduce At The Core Home Health and explain our available home-care services
- Build and maintain professional referral relationships
- Follow up with existing and prospective community partners
- Identify new referral opportunities throughout Durham Region
- Attend networking events, community events, health fairs, and local business events
- Distribute approved marketing materials
- Promote At The Core Home Health in a professional and ethical manner
- Schedule outreach meetings and presentations
- Conduct prospective client consultations when required
- Maintain accurate records of outreach activities, contacts, follow-ups, and results
- Submit a completed outreach tracker following each road day
Office Planning and Coordination
One day per week will be spent in the office preparing for outreach activities and supporting daily operations.
Responsibilities may include:
- Researching potential referral partners and community organizations
- Planning efficient travel routes for upcoming road days
- Scheduling outreach appointments and follow-up meetings
- Following up with referral sources by phone and email
- Updating outreach trackers, contact lists, and referral databases
- Preparing marketing packages and outreach materials
- Researching upcoming community events and networking opportunities
- Assisting with new client inquiries and intake coordination
- Supporting caregiver scheduling and shift coordination
- Assisting with client, family, and caregiver communication
- Preparing weekly activity reports and plans for the following week
- Supporting other administrative and coordination duties as required
Scheduling and Client Coordination
The successful candidate may also be responsible for:
- Coordinating caregiver schedules based on client needs and staff availability
- Responding to caregiver call-outs and scheduling changes
- Assisting with filling open shifts
- Communicating schedule updates to clients, families, and caregivers
- Supporting new client onboarding and service coordination
- Assisting with client consultations and intake processes
- Maintaining accurate client and employee documentation
- Escalating clinical, safety, staffing, or service concerns to management
- Providing professional customer service to clients, families, caregivers, and community partners
- Maintaining confidentiality and following privacy requirements
Every-Other-Weekend On-Call Support
The Care Coordinator will participate in the on-call rotation every other weekend.
On-call responsibilities may include:
- Responding to caregiver call-outs
- Managing urgent scheduling changes
- Assisting with shift coverage
- Responding to client and family concerns
- Communicating with caregivers regarding weekend issues
- Escalating urgent, clinical, or serious concerns to management
- Documenting all calls, concerns, schedule changes, and actions taken
- Remaining available and responsive throughout the assigned on-call period
Required Qualifications
Applicants must have:
- Previous experience in healthcare marketing, community outreach, business development, or referral relationship management
- Previous customer service experience, preferably in healthcare, home care, senior care, or a related field
- Previous scheduling, coordination, or administrative experience
- Strong verbal and written communication skills
- Strong organizational and time-management skills
- The ability to work independently and remain accountable for assigned goals
- Confidence speaking with healthcare professionals, community partners, clients, and families
- Strong problem-solving and decision-making abilities
- The ability to manage changing priorities and urgent situations calmly
- Strong computer skills, including email, spreadsheets, Microsoft Office, Google Workspace, and scheduling platforms
- A valid Ontario G driver’s licence
- A reliable vehicle and the ability to travel throughout Durham Region
- A satisfactory vulnerable sector check
- The ability to maintain confidential client and employee information
- Availability to participate in the every-other-weekend on-call rotation
Preferred Qualifications
The following would be considered assets:
- Experience working in home care, community care, senior care, nursing, or healthcare administration
- Experience using AlayaCare or similar home-care scheduling software
- Experience conducting client consultations or intake assessments
- Knowledge of Durham Region healthcare providers, retirement residences, community organizations, and referral sources
- Experience attending networking events and delivering presentations
- First Aid and CPR certification
- A diploma or degree in healthcare administration, business, marketing, social services, nursing, or a related field

The successful candidate must be comfortable travelling throughout Durham Region and managing their time effectively between outreach, planning, scheduling, and coordination responsibilities.
