As an Occupational Therapist – Community-Based Care, you will provide evidence-informed, client-centred occupational therapy services within clients’ homes and communities.
You will work collaboratively with clients, families, caregivers, referral partners, and interdisciplinary healthcare teams to improve function, reduce risk, promote independence, and support meaningful participation in everyday life.
Your referrals may include:
- Functional and home safety assessments
- Hospital-to-home transitions
- Falls prevention and aging in place
- Home accessibility and environmental modifications
- Equipment assessment and prescription
- Seating, positioning, and mobility
- Assistive Devices Program assessments and applications, where authorized
- Cognitive and functional assessments
- Community rehabilitation
- Personal care and attendant care assessments
- Client, family, and caregiver education
No Occupational Therapist is expected to practise across every service area described in this posting.
Referrals are thoughtfully assigned based on your demonstrated competence, experience, professional interests, applicable authorizations, geographic availability, and development goals.

Who We’re Looking For
We are looking for Occupational Therapists who combine compassion with sound clinical judgment and who genuinely enjoy helping people achieve greater independence.
Community experience is valued, but it is not the only path to success.

- Hospital Occupational Therapists interested in transitioning into community practice
- Rehabilitation and private practice clinicians
- Occupational Therapists with experience in geriatrics, neurology, orthopaedics, mental health, accessibility, seating and mobility, insurance, or complex care

Qualifications
- Degree in Occupational Therapy from an accredited or recognized program
- Current registration in good standing with the College of Occupational Therapists of Ontario
- Eligibility to practise occupational therapy in Ontario
- Professional liability insurance
- Strong communication, documentation, organization, and clinical reasoning skills
- Valid driver’s licence and access to reliable transportation for community visits, where required
Experience in any of the following areas will be considered an asset:
- Home and community care
- Ontario Health atHome
- Hospital discharge planning
- Community rehabilitation
- Home accessibility
- Seating and mobility
- Assistive Devices Program
- Cognitive assessment
- Geriatrics
- Neurological rehabilitation
Experience in every area is not required.
