Fife House is an innovative, client-focused provider of secure and supportive affordable housing and services for people living with HIV/AIDS in Greater Toronto. With approximately 110 staff working across 6 locations, in 2021-2022 Fife House served more than 945 residents and clients by providing supportive and transitional housing programs and outreach programs to homeless clients and their families.

ABOUT THE ROLE:
Fife House’s Linkage to Care program provides culturally-responsive and strengths-based case management to people living with HIV who have identified support needs relating but not limited to: physical health, medication access, mental health, substance use, criminal justice involvement, immigration, and/or precarious housing with the goal of improving housing stability. Homes for Good (HFG) is a subprogram of Linkage to Care. The Housing Stabilization Worker provides support to clients receiving the Homes For Good Subsidy and clients living within Fife House’s Housing Portfolio in partnership with St. Clare’s Multifaith Housing Society. The Housing Stabilization worker will provide both one-on-one and group-based support by engaging in regular follow up, facilitating referrals to services, and organizing activities for clients using a psychosocial rehabilitation approach to care. This position will work across multiple sites and be required to travel via municipal transit. This position will work directly with the Linkage to Care team, other Fife House staff, and other social service agencies.
DUTIES & RESPONSIBILITIES:
- Provide long-term coordination of care and housing stabilization support for clients living within Fife House’s Housing Portfolio in partnership with St. Clare’s Multifaith Housing Society and Toronto Community Housing across various locations;
- Coordinate support of portfolio with Housing Stabilization team and arrange visits on-site at each location daily;
- Complete intakes, assessments, goal setting, service plans, and discharge plans for eligible and enrolled clients;
- Conduct housing interviews for potential tenants with support from program manager, maintain active waitlist, and support new move-ins;
- Facilitate group programming at various locations, develop programs and activities focused on life skills development and social engagement, including inventory, planning, etc,;
- Gather client input on future programs and activities, through various methods such as surveys, groups, written suggestions, etc. and use in development;
- Facilitate and attend regular monthly meetings with housing coordinators at each location;
- Provide monthly follow up to low needs clients, bi-weekly to moderate needs clients, and weekly to high needs clients based on care plans and assessments;
- Refer residents to additional services as per their care plan, including HIV care, other medical services, mental health, employment, trusteeship programs, etc.;
- Provide intensive eviction prevention support, coordinate with landlord partners, and Landlord and Tenant Board;
- Prepare housing units for pest treatment and maintenance requests, and provide unit maintenance or cleaning support where housing is at risk;
- Directly assist with clients' activities of daily living as it supports housing stability and health maintenance, including the acquisition of life skills such as hygiene, medication assistance, cleaning, shopping and budgeting;
- Engage in crisis intervention/prevention with clients, safety planning, and provide supportive counselling;
- Facilitate and attend case conferences, housing support meetings, and care coordination meetings with resident’s care teams to promote housing and health stabilization;
- Participate in and facilitate Fife House’s CVITP tax clinic and complete taxes throughout the year for residents and eligible clients;
- Complete Rent-Geared-to-Income and/or Annual Review documentation with clients on a yearly basis and coordinate related documentation;
- Maintain accurate and timely reporting data as per program policies and procedures, and partnership and funding criteria, as per the Linkage to Care program manager;
- Document client-related and care coordination activities in the online case management system in an accurate and timely manner;
- Maintain accurate and updated client and program records;
- Participate in team meetings, case management rounds, supervision, and professional development;
- Conduct Office Hours at program location on a rotational basis as scheduled;
- Submit expense claims and relevant documentation in accordance with agency regulations;
- Will be required to enter clients’ units and may be required to lift heavy items;
- May be required to work evenings and/or weekends, or schedule may change as per program needs;
- Participate in program partnership with the Blue Door Clinic and provide time-limited case management services as directed;
- Participate in program evaluation and development activities, and liaise with agency community partners;
- Provide co-supervision to placement students with the Program Manager;
- Other duties as assigned by the Program Manager or management team;
Qualifications:
- A post-secondary diploma/certificate in Social Services (i.e. SSW, BSW) or equivalent with relevant experience in the housing and homelessness sector; or equivalent work and lived experience;
- Ability to use public transportation and/or walk to various locations on a daily basis in all weather conditions;
- Ability to lift heavy items on occasion, up to 25kg (~55lbs), with safety equipment provided when necessary;
- Knowledge of income and housing systems in the City of Toronto;
- Ability to plan, organize and coordinate activities
- Demonstrated ability in the delivery of client assessment tools, goal setting, service planning and care coordination;
- Demonstrated experience in working within a multi-disciplinary team, as well as independently;
- Highly developed understanding of and proven ability to communicate appropriate professional boundaries;
- Knowledge of the Residential Tenancies Act, Ontario Social Housing Act, and the Ontario Accord (GIPA/MEPA);
- Strong written skills, oral skills, and computer skills (Microsoft 365, MS Word, Excel, Power Point, Internet, MS Publisher, etc.);
- Excellent organizational (detail-oriented) and time management skills;
- Excellent written and oral communications skills;
ASSET:
- Knowledge of community resources for HIV/AIDS, mental health and addiction,
- Knowledge of HIV/AIDS and related health comorbidities, addictions and mental health issues experienced by Black, Indigenous and Persons of Colour, and/or 2SLGBTQIA+ communities;
- housing/eviction prevention, the criminal justice system, and immigration Non-Violent Crisis Intervention, First Aid and CPR training, Mental Health First Aid, and Naloxone training;
- Developed conflict resolution and negotiation, crisis intervention/prevention skills;
- Knowledge of anti-racist, culturally-responsive practices, psychosocial rehabilitation principles, and a trauma-informed lens;
- Lived experience of HIV, mental health, substance use, and/or homelessness;
- Identify as Black, Indigenous, and/or a Person of Colour;
- Additional spoken languages;
