For over 35 years, The Safehaven Project for Community Living has been providing exceptional care to children with developmental disabilities and complex care needs.

Our mission is to provide high-quality, family-centered, and community-based residential and respite care for individuals with complex care needs through respite and residential programs at Toronto, Etobicoke, North York, Woodbridge and Aurora homes.

ROLE SUMMARY

The Occupational Therapist (OT) provides client-centered assessment, intervention, and consultation services to medically complex clients to enhance functional independence in self-care, productivity, and leisure activities. Working in partnership with clients, families, and the interprofessional team, the OT develops and implements individualized, measurable care goals and addresses occupational performance challenges related to physical, cognitive, and environmental factors.

The OT delivers evidence-informed care, provides education to families and staff, and supports the appropriate use of adaptive equipment and mobility devices. The role includes documentation in accordance with organizational and College standards, participation in quality improvement initiatives, and promotion of a safe, client-centered care environment.

RESPONSIBILITIES:

Partner with clients and families to formulate and implement individualized client-centered goals with focused, targeted and measurable outcomes.
Provide client care in collaboration with the client/family to plan, implement and evaluate care, documenting the habilitative/rehabilitative process according to Safehaven’s policies.
Interface and problem solve with members of the interprofessional care team and community providers as appropriate to ensure an integrated approach to care.
Incorporate evaluative activities into professional practice to ensure the delivery of efficient and effective health care.
Demonstrate strong critical thinking/reasoning and utilize research to establish and support evidence-informed practice.
Utilize the teaching/learning process to provide education on various conditions, both formal and informal, to clients/families, colleagues, students, and other professionals within the organization and the community.
Provide direct service, consultation, and education to clients, families, and community providers with focus on maximizing the client’s function in the areas of self-care, productivity (school and play skills) and leisure (community or sports participation).
Address challenges identified in client occupational performance. Provide assessment and intervention directed at the person, occupation and environmental factors influencing occupational performance such as activities of daily leaving; transfer and lift devices; wheelchairs and seating, etc.
Practice according to organizational policies and standards of practice as set out by the College of Occupational Therapists of Ontario and the policies, procedures and standards of care of Safehaven.
Participate in quality improvement initiatives, supports the maintenance of a safe and healthy work environment and advance a culture of client/patient safety through work and daily practices.

QUALIFICATIONS:

Bachelor's degree in Occupational Therapy or Master's degree in Occupational Therapy
Current registration with the College of Occupational Therapists of Ontario
Membership in the Canadian Association of Occupational Therapists and/or the Ontario Society of Occupational Therapists is an asset.
Knowledge of and experience in occupational therapy assessment and treatment of children and youth with a variety of conditions
Knowledge of and experience with occupational therapy assessment techniques, therapeutic skills, modalities and their applications
Knowledge and experience with a wide variety of lifts and transfers from wheelchairs, beds, bathtubs, toilets, etc. required
Experience with current outcome measures (i.e. Canadian Occupational Performance Measure, Goal Attainment Scaling, manual muscle testing, etc.)
Demonstrated ability to manage fluctuating clinical responsibilities effectively and efficiently
Demonstrated ability to communicate effectively and in a solution-focused manner with clients, families and members of the interprofessional care team
Proven ability to work independently and collaboratively as a member of an interprofessional team is essential
Demonstrated commitment to continued professional development
Experience with being a positive change agent and engagement in quality improvement initiatives is an asset
