Summary of Role:
Palliative Pain and Symptom Management Consultants (PPSMCs) provide secondary-level hospice palliative care expertise through consultation, coordination, collaboration, facilitation, education, skill development, research, and evaluation at the local, regional, and provincial levels.
PPSMCs work across the full continuum of care, supporting health service providers in a variety of settings, including but not limited to primary care, long-term care, retirement homes, NSM OHaH, hospices, and community nursing/support agencies.
They are committed to advancing best practices by providing resources and education that equip interprofessional care teams to deliver a palliative approach to care for individuals of all ages living with life-limiting illnesses.
Required Qualifications and Experience:
- Bachelor’s degree in nursing, registered in good standing with the College of Nurses of Ontario
- Canadian Nurses’ Association Certification in Hospice Palliative Care (CHPCN(C))
- Minimum of 3 – 5 years’ experience practicing in palliative care
- Successful completion of Comprehensive Advanced Palliative Care Education (CAPCE) preferred
- Possession and maintenance of a valid ON Driver’s License and vehicle insurance with access to a reliable vehicle and an ability to drive within the North Simcoe Muskoka Area
Clinical consultation and Education Responsibilities:
- Provide consultation and palliative care and pain and symptom management to service providers to assist with the assessment and management of pain and other symptoms
- Help to build capacity among front-line providers in the delivery of palliative care through skill development while driving knowledge to action across the continuum of care
- Provide care-based education and mentoring for service providers
- Link service providers with specialized hospice palliative care resources
- Assist service providers in the use of assessment tools and best practice guidelines for the application of a palliative approach to care through consultation and education
- Develop and implement mentoring programs across sectors with a focus on Long-Term Care Homes, Retirement Homes and Primary Care
- Provide formal/informal education and mentoring for health care providers to build capacity among front-line health service providers
- Facilitate courses including Fundamentals of Hospice Palliative Care, Advanced Palliative Practice Skills (APPS), Comprehensive Advanced Palliative Care Education (CAPCE), Essentials In Pain Management (EPM), Canadian Serious Illness Conversations (CSIC), Learning Essential Approaches to Palliative Care (LEAP)
- Develop and cultivate rapport to build and foster trust and credibility with partners and the wider community
- Support the Program Manager with the planning and execution of strategic initiatives
- Potential participation in local, regional and provincial-level committees/ projects
Core Skills and Competencies for the Role
- Advanced clinical assessment and critical thinking skills, with in-depth knowledge of life-limiting illnesses and a palliative approach to care, including end-of-life care
- Strong education and facilitation expertise, with the ability to teach and engage adult learners using in-person, virtual, and blended approaches, and adapt to diverse learning styles and environments
- Excellent communication skills, both written and verbal, with the ability to convey complex information clearly, using tact, diplomacy, and professionalism
- Comprehensive understanding of the healthcare system across sectors, including OHaH, acute care, long-term care, primary care, and community services
- Highly developed organizational, project management, and prioritization skills, with the ability to coordinate complex initiatives and manage competing demands in fast-paced environments
- Proficient in the use of digital tools and platforms, including electronic medical records, learning management systems (e.g., D2L Brightspace), and Microsoft Office applications
- Self-motivated, adaptable, and compassionate, with a strong commitment to continuous professional development

Work Environment:
- This role operates across multiple care sectors and includes a combination of remote, home-based work and travel to various sites for in-person education, collaboration, and meetings. The position requires regular use of standard office technology and virtual platforms (e.g., Microsoft Teams, Zoom) to support communication, education, and coordination activities.
- The role functions within a dynamic and evolving environment, requiring adaptability, collaboration, and engagement with interdisciplinary teams and community partners. - This position involves extended periods of sitting and frequent computer use, with variable workflow requiring flexibility, adaptability, and effective time management
- Requires the ability to communicate effectively, including speaking, listening, and interpreting information
- Requires close visual focus and the ability to adjust focus as needed for computer and document-based work
- Occasional lifting of up to 25 lbs may be required, along with tasks such as reaching, bending, or using a step stool
Position Details & Travel Requirements:
- This is a full-time, permanent position (1.0 FTE), scheduled Monday to Friday (37.5 hours per week).
- The role includes participation in the Healthcare of Ontario Pension Plan (HOOPP), a cost-shared pension plan
- Optional eligibility for extended health benefits through RWAM.

Candidates must have access to a reliable vehicle and a valid Ontario driver’s license. This position requires travel within NSM. Mileage is reimbursed in accordance with organizational policy and submitted monthly.
