Blood Ties is looking for two part-time Health Care Providers (HCP) at the Supervised Consumption Site (SCS).
Blood Ties Four Directions Centre is inviting applications from nurses and paramedics who are seeking part-time employment as Health Care Providers. Blood Ties operates the Yukon’s first Supervised Consumption Site (SCS) in partnership with Mental Wellness and Substance Use Services.
The SCS provides a safe and confidential space for people to consume their personal drugs with one of the only smoking rooms in Canada, using state-of-the-art filtration technology to provide a safe environment for employees and clients. Community members can also access wrap-around services at the site, including harm reduction education and support, referrals, nutrition support, drug-checking services, and harm reduction supplies.
Roles and Responsibilities:
Under the leadership of the Team Lead, the Health Care Provider (HCP) HCP— SCS (Supervised Consumption Site) works collaboratively as a member of the Harm Reduction team that operates within the structure outlined in the Supervised Consumption Site Staff Roles and Responsibilities document. Within this context, the HCP-SCS provides low-barrier medical care based on harm reduction principles within their legislated scope of practice, standards of practice, organizational policies and procedures and competencies.
Within this scope of practice, the HCP position provides care to marginalized SCS clients in a culturally appropriate, patient centric and trauma informed manner. This could include but not be exclusive to: wound care, mental health care including suicide risk assessment and chronic disease management.
The HCP is responsible as a member of the SCS team for delivering services to individuals accessing the SCS who may present with complex care needs, including physical, cognitive and developmental disabilities, mental health and substance use challenges, and social isolation in a welcoming, safe and hygienic environment. When present, the HCP is equipped to take the lead in providing initial risk assessment for clients presenting with suicidal ideation, providing de-escalation support and making appropriate referrals and/or calls to appropriate emergency response supports.
This position provides a strong focus on prevention, education and health promotion. The HCP will take responsibility for keeping an up-to-date inventory of all first aid supplies within the SCS and will create orders for replacement supplies and resources on a monthly basis. The HCP may also be involved in referral to medical assessment, chronic disease management, coordination and support within the services and activities of Mental Wellness and Substance Use Branch.
The HCP-SCS utilizes sound problem solving and crisis management skills and holds knowledge of how to leverage community resources to meet client health and safety needs, while responding to clients’ concerns and complaints from a Health Care Provider’s lens. The HCP provides medical support to clients experiencing infections or injuries related to the use of substances, including referrals to and collaboration with other healthcare providers and is in a position to provide leadership in delivering harm reduction education to clients including safe consumption techniques, substance use, sexual health and any other health concerns brought forward by clients
Professionalism is maintained in the HPC role through collaboration with colleagues, continuous professional development and participation in quality improvement processes as is appropriate. Completion of an annual Professional Learn Plan is required.
