Job Summary
The Ancient Rivers Family Health Team (FHT) is a group of interprofessional healthcare providers associated with the Ancient Rivers Family Health Organization (FHO), a group of 17 family doctors in Renfrew and Calabogie with a combined roster of 14 000 patients on 1 shared EMR (Telus Practice Solutions). The Registered Dietitian (RD) will work closely with our Primary Care Providers and Integrated Healthcare Providers as part of a dynamic interdisciplinary health care team. The RD provides medical nutritional therapy to the patients of the FHT and participates in the development, implementation, monitoring, and evaluation of the FHT’s programs and services. This is a temporary position until March 31, 2027. We are open to individuals interested in full time or part time hours.
Main Responsibilities
· Provide direct patient care, with a focus on chronic disease management
· Conduct assessments and provide consultations to patients that are referred to the dietitian program either in person, or virtually
· Develop and deliver education to patients, other team members and community partners pertaining to nutrition and health
· Provide education to patients in a way that best meets their individual learning abilities and that optimizes patient encounters
· Incorporate principles of Patient Self-Management into every patient visit
· Collaborate with Physicians/Nurse Practitioners and other interdisciplinary health care professionals (IHPs) to make decisions regarding delivery of services to patients, and to facilitate patient-centered care
· Communicate effectively with physicians/NPs to provide information pertaining to patient’s care plan
· Utilize the electronic medical records system (EMR) to record and review information pertaining to patients that can be used for patient assessments and consultations
· Collaborate with Administrative Assistants as needed to ensure optimal scheduling for patients, and to provide input into the scheduling process
· Lead, participate and provide input into a variety of team meetings, projects and committees, as requested
· Assess and make recommendations regarding nutrition services to ensure it aligns with best-practices and the ARFHT’s strategic direction
· Connect and build relationships with community partners, as needed
· Input statistical information through an established process that is aligned with the MOHLTC reporting requirements
· Update and maintain nutritional resources to ensure up to date and best-practice information is provided to patients
Position Qualifications and Skills
· Bachelor Degree in Nutrition and Dietetics or similar; graduate of an accredited Dietetic Internship program
· Registered and in good standing with the College of Dietitians of Ontario
· Membership with Dietitians of Canada an asset
· Current Certified Diabetes Educator (CDE) an asset
· 3 years related or applicable experience in a Community and/or Primary Health Care setting an asset
· Experience in program development and evaluation an asset
· Demonstrated ability to communicate effectively with patients, families, and members of an interdisciplinary medical team.
· Excellent written and verbal communication skills
· Intermediate knowledge of Microsoft Office Suite and EMR (e.g. Telus Practice Solutions Suite)
· Experience in patient education and chronic disease management.
· Demonstrated ability to work well independently and in a team environment
· Current Ontario Driver’s License and access to a vehicle for travel within the region

Pay: $36.34-$42.51 per hour
