The Clinical Care Coordinator reports directly to the Director of Care and serves as a coach and mentor to all employees of the nursing department, leading by example to inspire and motivate everyone on the team.

THE RESPONSIBILITIES
- Provide supportive, effective leadership to all employees of the nursing department.
- Provide strong, consistent leadership to ensure the maintenance of a cohesive and cooperative management team.
- Ensure that effective management systems, including reporting and internal control, are maintained to ensure compliance with all applicable legislation, the Fixing Long Term Care Act and all Omni standards, policies and procedures.
- Ensure appropriate management presence and nursing leadership are available in the home. Recognize that hours and days of work may be altered from time to time, based on the needs of the home. Assume on-call responsibilities as directed by the Administrator and/or Director of Care.
- Monitor supplies and maintain inventory system for nursing and continence supplies. Order, receive and ensure appropriate rotation and storage of nursing supplies. Ensure adequate supplies are available based on the needs and inventory requirements of the Home.
- Assist the DOC to ensure that documentation in the clinical record is accurate, current and complete. Coordinate and implement activities to ensure that care and funding requirements are met; assist in the facilitation of processes required to ensure that MDS assessments and RAPs are an accurate reflection of the residents and the care that they are provided in the home.
- Administer medications as required and provide direct resident care in accordance with the day to day needs of the home, including assessment, wound care, documentation, processing orders, liaison with Physicians, communication with families, etc.
- Facilitate effective and supportive relationships with representatives of the Ministry of Long Term Care, HCCSS, Accreditation Canada, community partners and other external agencies and organizations.
Education
- A Bachelor of Science degree in Nursing OR
- Diploma in Nursing, as well as a certificate of competence as a Registered Nurse
- Registered Practical Nurse with the College of Nurses of Ontario.
Experience
- At least one year of experience working as a registered nurse or registered practical nurse in the long term care sector.
- At least three years of experience working as a registered nurse or registered practical nurse in a managerial or supervisory capacity in a healthcare setting.
