Job Overview
We are seeking a compassionate and dedicated Registered Practical Nurse (RPN) to join our healthcare team. The RPN will play a vital role in providing high-quality patient care in various settings, including home care, nursing homes, assisted living facilities, and pediatrics. The ideal candidate will possess strong clinical skills and a commitment to improving the health and well-being of our patients.
Responsibilities
- Deliver direct patient care, including administering medications and treatments as prescribed.
- Conduct assessments and develop individualized care plans for patients.
- Monitor patients' conditions and report any changes to the healthcare team.
- Collaborate with physicians, nurses, and other healthcare professionals to ensure comprehensive patient care.
- Provide education and support to patients and their families regarding health management and treatment options.
- Maintain accurate patient records using electronic health record systems such as Cerner.
- Assist with case management duties to coordinate care across various services.
- Ensure compliance with safety protocols and infection control measures.
Skills
- Proficient in home care practices and case management strategies.
- Experience working in nursing homes or assisted living environments is preferred.
- Familiarity with acute care settings is advantageous.
- Previous experience in pediatrics or caring for children is a plus.
- Strong patient care skills with the ability to build rapport with diverse populations.
- Excellent communication skills, both verbal and written, for effective interaction with patients, families, and team members.
- Ability to work independently as well as part of a collaborative healthcare team.
- Knowledge of electronic health record systems, particularly Cerner, is desirable.
