Job Summary
We are seeking a compassionate and dedicated Registered Practical Nurse (RPN) to join our healthcare team of Family Practice . The ideal candidate will provide high-quality patient care across various settings, including primary care This role requires a skilled professional with strong clinical knowledge and excellent communication skills to support patient well-being and recovery. The RPN will collaborate with multidisciplinary teams to deliver comprehensive nursing services, ensuring safety, comfort, and dignity for all patients.
Responsibilities
- Deliver direct patient care, including assessments, injections administration, and helping in treatment procedures
- Monitor patient health status and document observations accurately using EMR ( Telus PS Suite )
- Assist with specialized care needs such as dementia care, pediatrics, or senior care as required
- Perform phlebotomy and laboratory specimen collection in accordance with protocols
- Manage pain control and medication management plans in collaboration with physicians and other healthcare professionals
- Educate patients and families on health conditions, treatments, and post-discharge care instructions
- Maintain compliance with healthcare regulations and safety standards to ensure a safe environment for patients and staff
- Participate in case management activities to coordinate ongoing patient care plans
- Support discharge planning and transition of care processes for optimal patient outcomes
Qualifications
- Valid registration as a Registered Practical Nurse (RPN) with the appropriate licensing body
- Prior experience working in primary care settings preferred
- Knowledge of acute care , pediatric , chronic illness management or antenatal care is advantageous
- Proficiency in medical terminology and medication administration protocols
- Experience with electronic medical records ( Telus PS Suite ) is desirable
- Strong communication skills with the ability to work effectively within multidisciplinary teams
- Ability to handle sensitive situations with professionalism and compassion
- Additional skills such as phlebotomy, laboratory experience, pain management expertise, or case management are considered assets
