Supervisory Responsibilities (including but not limited to):
- Supervising daily activities of assigned care team and providing medical guidance as needed
- Working scheduled shifts with clients in assigned pod to get a full scope of care needs, training requirements and caregiver qualifications needed
- Creating and developing care plans for assigned clients within pod, with assessments conducted regularly to ensure clients receive the care they need
- Collaborating closely with Client Director to adjust care plans, discuss resources needed for clients within each pod
- Administrative Duties (reviewing caregiver charting notes for clients within pod, communicating any changes, answering phone calls and emails)
- Collaborating with other health care professionals in the community regarding client, family and staff concerns
- Determines assistance, equipment or information required by the client or family
- Responding to medical questions and concerns
- Included in a rotation of monitoring text line during evenings and weekends (including but not limited to: providing medical guidance, updating client and family with any schedule changes etc.)
- Providing input into the assessment process to ensure clients receive appropriate services
- Completing all necessary charting, paperwork, and reporting in a clear, concise, and timely manner using electronic charting
- Training new Caregivers for new clients in pod, or clients requiring extra care
- Providing support and supervision to designated staff and providing effective communication strategies, conflict resolution, performance recognition/feedback, and safe work practices
Care Responsibilities:
- Assisting with personal hygiene (bathing, grooming, dressing, self care etc.)
- Medical appointment assistance
- Planning assistance (meals/groceries/errands)
- Assisting with bed transfers, lifting, and turning
- Personal/Accompanied errands
- Preparing/serving meals
- Providing wound care
- Household tidying & light house cleaning
- Coordinating both indoor & outdoor activities
- Daily companionship (play games, conversations, puzzles, etc.)
- Helping with technology/or connecting via technology with family or friends
- Helping with personal admin (including financials, medical, taxes, pay bills, funding, etc.)
- Additional services upon request based on client needs
Qualifications:
- Current Licensed Practical Nursing Diploma or certificate
- At least 2-3 years of experience as an LPN
- Valid Class 5 Driver’s License, and reliable transportation
- Clean Drivers Abstract
- Criminal Background/Vulnerable Sector Background Check required
