Duties
- Assist healthcare providers with patient care activities, including data collection and documentation
- Manage electronic medical records using systems such as Epic and Cerner with accuracy and confidentiality
- Support clinical procedures such as phlebotomy and medication administration under supervision
- Document patient histories, vital signs, and clinical observations in electronic systems
- Aid in research activities related to patient outcomes and clinical studies
- Prepare examination rooms and assist during patient visits in outpatient or home care settings
- Support hospital staff with administrative tasks related to patient flow and record keeping
- Ensure compliance with healthcare protocols and safety standards in acute care, pediatrics, and pain management settings
Qualifications
- Internationally educated MD with valid medical licensure or certification recognized locally
- Experience working in hospital or clinical environments, preferably with exposure to acute care, pediatrics, or pain management
- Proficiency with electronic medical records systems such as Epic and Cerner
- Knowledge of medical terminology and clinical procedures including phlebotomy and medication administration
- Strong organizational skills with attention to detail in data collection and documentation
- Excellent communication skills for effective collaboration within multidisciplinary teams
- Ability to adapt quickly to new environments and work efficiently under supervision
- Prior experience in research or healthcare data management is a plus
- Demonstrated commitment to patient care, safety, and confidentiality

The Intake Coordinator is responsible for coordinating admissions and managing the intake process for clients entering NAM Recovery programs. This position ensures timely completion of assessments, intake documentation, medication coordination, communication with referral sources and external partners, bed management, and client move-in requirements. The Intake Coordinator supports client safety, maintains accurate records, coordinates admissions and follow-up activities, and assists with outreach, presentations, and program operations as required.
- Must be fluent in both Punjabi and English
- Excellent English communication and writing skills are required.
Primary Responsibilities Intake and Admission Coordination
- Manage intake calls, callbacks, and email inquiries.
- Complete phone and in-person intake assessments.
- Coordinate admissions and move-in planning.
- Maintain the waiting list and bed availability.
- Track upcoming discharges and planned admissions.
Safety and Clinical Screening
- Complete suicide risk and safety assessments.
- Develop safety plans upon admission.
- Review Netcare information for safety concerns.
- Ensure medical assessments are completed within required timelines.
- Communicate concerns to physicians, pharmacists, managers, and clinical staff.
Medication and Medical Coordination
- Collect medication histories and enter information into Med Access.
- Coordinate medications before admission.
- Arrange urgent medical appointments when necessary.
- Communicate medication concerns with medical providers.
- Support transportation to urgent medical appointments when required.
Documentation and Record Management
- Complete intake notes, move-in notes, and follow-up notes.
- Ensure all admission forms and residency agreements are completed.
- Review documentation completed by other staff for accuracy.
- Maintain professional records and ensure timely filing.
Communication and External Partnerships
- Communicate with hospitals, detox centres, lawyers, remand centres, pharmacies, doctors, and referral agencies.
- Maintain professional communication through designated NAM email accounts.
- Support outreach and community presentations.
Client Follow-Up and Support
- Complete first five-day follow-up contacts.
- Monitor client adjustment and support needs.
- Report safety concerns and incomplete information to management.
Urine Testing Coordination
- Ensure urine testing requirements are completed.
- Document testing appropriately.
- Report missed or concerning results to management.
Data Reporting and Program Support
- Prepare weekly admission and bed occupancy reports.
- Participate in data presentations and management meetings.
- Support PRP programming and administrative activities as required.
Evening and Weekend Intake Support
- Respond to urgent admission needs outside regular hours when required.
- Obtain missing information and coordinate urgent admissions.
Qualifications
- Diploma or degree in Addictions, Social Work, Human Services, Psychology, Nursing, or a related field preferred or International educated MD or Nurse preferred, Read the Wages before apply kindly.
- Experience in addictions, mental health, residential treatment, or community support services preferred.
- Strong organizational, communication, and documentation skills.
- Ability to manage multiple priorities and maintain confidentiality.
- Knowledge of Med Access, Netcare, and Microsoft Office considered an asset.
- Valid driver's license and reliable transportation preferred.
- Ability to work occasional evenings and weekends as required.
Schedule
- Full-Time or Part-Time (0.4–1.0 FTE)( Tuesday -Thursday required ,Not flexible)
- Monday to Friday
- Evening and weekend coverage as required
