We are seeking a compassionate, creative, and reliable Community Inclusion Support Mentor to support our family member in building a meaningful and purposeful adult life.
Reporting to the family, this role focuses on facilitating participation in community activities, building friendships, developing a structured daily routine and independence with daily living skills, and finding volunteer opportunities with the potential for part-time employment. This is a part-time position for approximately 30 hours per week.
About the Role
The person being supported is a thoughtful, observant, and detail-oriented young adult who is happiest when they have meaningful activities scheduled and opportunities to contribute. They have a strong passion for animals, crafting, pop culture, and HGTV style videos. As they continue to grow, their goals include exploring community, volunteering and building independence.

Key Responsibilities
- Build a positive and respectful relationship with the person
- Provide attentive, person-centred supports tailored to their needs
- Plan, coordinate, and participate in community-based activities
- Identify and facilitate opportunities for community engagement, including volunteer and employment opportunities
- Encourage social interactions, inclusion and companionship with others
- Support skill development and independence through daily activities and experiences
- Support establishing and strengthening healthy habits and self-care routines
- Follow and implement safety plans in home and community settings
- Work independently while maintaining open communication with the family
- Communicate concerns promptly and professionally
Qualifications & Requirements
- Strong interpersonal and communication skills
- Reliable, dependable, and highly organized
- Demonstrated understanding of inclusion and community engagement
- Valid class 5 Driver’s Licence with a reliable vehicle
- Clear Criminal Record Check with Vulnerable Sector
Assets
- Experience supporting individuals with intellectual disabilities or diverse support needs
- Experience in one or more of the following
- Psychology
- Education
- Social work
- Disability services
- Autism support
- Occupational therapy
- Recreational therapy
- Community support work
- First Aid/ CPR certification
