Compensation: $26.37- $34.22 Hourly

Position Summary:
The Intensive Addictions Care Manager (IACM) will engage clients and caregivers, particularly those whose lives have been impacted by substance use/concurrent disorders, in a collaborative process to manage their health (physical, mental and social). The IACM will be responsible for initial and ongoing assessment, care planning, intervention and making ongoing adjustments as required. Interventions include making referrals, advocacy, life skill enhancement, personal care coaching, self-efficacy promotion, and collaboration with other service providers.
Responsibilities:
- Manage a caseload of clients, including supporting individuals living with substance use concerns in the community as well as those involved within the Addiction Supportive Housing program.
- Assist client and remove barriers to obtaining internal and external supplementary supports or services to sustain independent living (e.g., transportation, income maintenance, health care, food management, leisure).
- Supporting clients to make social connections and integrate with neighbors and the local community
- Conduct standardized assessments, if appropriate, using approved tools/ documents
- Conduct life skills assessment to determine client’s specific needs to enhance independent living capabilities and promote self-efficacy (e.g. money management, food preparation, fire safety, housekeeping, etc.)
- Develop Care Plans and Provide Interventions.
- Negotiate and develop individualized care plans, based on assessment results, client’s strengths, prioritized goals, client’s preference and readiness, clinical judgement and identification of potential barriers to treatment entry.
- Conduct individual counselling sessions, including early intervention, relapse prevention, crisis intervention, as well as lifestyle and personal counseling to assist individuals to self-care
Education:
- Minimum post-secondary education in the field of substance use/addictions.
- Membership in good standing with the Ontario College of Social Workers and/or Social Service Workers (OCSWSSW) an asset
- Valid First Aid and CPR certificate an asset
Skills & Experience:
- 1-2 years’ experience providing direct clinical care focusing on substance use and concurrent disorders
- Minimum of 1-year experience in community development and networking to facilitate referrals both to and from the organization
- Experience conducting intake and structured assessments for services
- Experience facilitating individual and group treatments as well as ongoing care/case management
- Broad knowledge of addictions treatments and explanatory theories, homelessness, mental illness, and community resources
- Adequate understanding of community resources (social, legal, health and financial)
- Experience working with older adults and seniors, an asset
- Excellent interpersonal, communication, and decision-making skills
- Excellent organizational and time management skills
Working Conditions:
- Required to travel to conduct home/community visits with clients and/or caregivers; working alone in clients’ homes
- Required to work occasional evenings and weekends
- Regular interruptions to support clients’ needs and crises
- Regular exposure to computer terminal
- May be exposed to communicable diseases

- A valid driver’s license and a car are required for home visits
