Contract – 12 Months (30 Hours / Week)
Community Care City of Kawartha Lakes (CCCKL) is seeking a Clinical Coach – Palliative Care to join our Hospice Services team. This position supports the implementation of Ontario Health’s palliative care priorities across community, long-term care, and hospital settings through clinical coaching, education, mentorship, and quality improvement activities.
This is a 0.8 FTE maternity leave contract position anticipated to start in August 2026 and continue until October 2027. The role is currently funded through Ontario Health, with confirmed funding to March 31, 2027. Funding for the period beyond March 31, 2027 remains subject to Ontario Health approval.
THE ROLE:
The Clinical Coach works collaboratively with Ontario Health regional palliative care partners and local health service providers to strengthen palliative care delivery across the City of Kawartha Lakes and Haliburton County. The role focuses on building capacity among front-line providers, supporting earlier identification of palliative care needs, facilitating goals of care conversations, and improving transitions between care settings.
The overarching goals of the Clinical Coaching initiative are to:
- Support Long‑Term Care Homes (LTCHs) to improve quality of life for residents and enable residents to die in their preferred place of death through strengthened palliative approaches to care including early goals of care conversations.
- Strengthen and increase palliative care delivery in the home setting by enabling earlier identification of palliative care needs and care planning that is in alignment with patients’ preferred place of death.
- Strengthen transitions in care across Hospital and Community Settings to support patients with palliative care and/or end-of-life care needs to receive care and die in their preferred place of death.
KEY ACCOUNTABILITES:
- Support implementation of Ontario Health’s Adult Models of Care for palliative care.
- Coordinate and deliver palliative care education and coaching aligned with Ontario Health priorities and the Adult Models of Care.
- Provide palliative care mentoring for health service providers in the long-term care, community and hospital sector settings in alignment with the Ontario Palliative Care Competency Framework.
- Build competency in early identification of palliative care needs and goals of care conversations.
- Collaborate with regional partners, healthcare organizations, and community agencies to improve access to palliative care.
- Support quality improvement and change management initiatives related to palliative care delivery.
- Participate in data collection and reporting activities in alignment with Ontario Health expectations.
- Contribute to the development and sharing of palliative care resources, education sessions, and community outreach initiatives.
KEY COMPETENCIES:
- Regulated health professional in good standing (RN, RPN, NP, or RSW).
- Minimum of 5 years of palliative care experience.
- Demonstrated experience in clinical coaching, education, mentorship, and interdisciplinary collaboration.
- Strong knowledge of palliative care best practices, goals of care conversations, and community-based service delivery.
- Excellent communication, facilitation, and relationship-building skills.
- Ability to work independently and collaboratively across multiple organizations and sectors.
- Valid driver’s licence and access to reliable transportation.
COMPENSATION:
The wage range for this exciting opportunity is $39.09 – $42.80. Selected candidates will be appropriately placed on the organizations wage grid based on skill, knowledge and experience.
