Job Overview
We are seeking a compassionate and dedicated Licensed Practical Nurse (LPN) to join our healthcare team. The ideal candidate will provide high-quality patient care across various settings, including nursing homes, hospitals, assisted living facilities, and home care environments. Responsibilities
- Deliver comprehensive patient care by monitoring vital signs, administering medications, and assisting with daily activities.
- Perform clinical tasks such as phlebotomy, wound care, and pain management under physician orders.
- Document patient information accurately using electronic health records systems
- Assist with patient assessments, develop care plans, and participate in case management activities.
- Support patients with dementia, hospice needs, or chronic conditions in various settings including assisted living and home care.
- Collaborate with physicians, nurses, and other healthcare professionals to ensure coordinated and effective patient care.
- Educate patients and their families on health management, medication use, and post-discharge instructions.
- Maintain compliance with healthcare regulations and safety protocols to ensure a safe environment for patients and staff.
Skills
- Proven experience in nursing home, hospital, or acute care settings.
- Expertise in dementia care, pediatrics, senior care, hospice care, or home health is highly desirable.
- Proficiency in medication administration, medical terminology, and laboratory procedures such as phlebotomy.
- Familiarity with electronic health records systems including Epic and Cerner.
- Strong patient care skills with an emphasis on compassion and attention to detail.
- Ability to work effectively within a team environment while managing multiple priorities.
- Excellent communication skills for interacting with patients of all ages and their families.
