PRIMARY FUNCTION:
Under the direction of the Program Manager, Addiction Care, or delegate, the Registered Practical Nurse (RPN) is accountable to the public and responsible for ensuring that practice and conduct meets legislative requirements and the standards of the profession, as defined by the College of Nurses of Ontario. RPNs’ practice will also comply with Kingston Health Sciences Centre’s policies, procedures, and clinical standards. The RPN autonomously provides care within a full scope of practice as outlined in the College of Nursing Standards of Practice. The R.P.N. will adhere to worker responsibilities as set out in the Occupational Health & Safety Act, hospital safety policies, and dept/unit established procedures at all times. Within this role the employee is accountable for contributing to the delivery of the KHSC strategic direction and demonstrates a commitment to patient and family centred care, guided by the core values of dignity, respect, information sharing, participation, and collaboration.
Working as a member of the interdisciplinary Detoxification Centre Team, the RPN provides nursing, withdrawal management, and addiction care to individuals to detoxify from alcohol and/ or other substances. The RPN collaborates closely with clients, families, addiction care workers, physicians, nurse practitioners, and community partners to promote safe, person centred stabilization and transition planning. Care is provided in accordance with harm reduction principles, trauma informed practice, and cultural safety, ensuring a respectful and supportive environment for clients in all stages of recovery.
The RPN participates in quality improvement, risk management, and health and safety initiatives, while modeling professional behavior that fosters team work, compassion, and excellence in addiction care.
PRINCIPLE RESPONSIBILITIES:
Patient/Family Care
The RPN, as a member of the interdisciplinary team, and using the best available evidence, will assess, plan, implement, and evaluate the nursing care delivered patients and families within the RPN scope of practice including outcomes, specific interventions and modifying the plan of care, accordingly.
- Prioritizes patient care based on individual patient needs
- Recognizes deviations from predicted patient responses and consults appropriately
- Reports all changes in patient conditions, monitoring the changes with accuracy, and continuing to consults appropriately with the NP/ Physician and other health care team members.
- Develops plans of care and coordinates care to achieve identified patient goals
- Meets immediate identified nursing care needs
- Assigns, teaches and supervises unregulated care providers (UCP)
- Teaches and delivers elements of established health programs to meet the bio/psycho/social, cultural, emotional and health learning needs of the patient and/or family
- Exhibits critical thinking within the RPN scope of practice
Team Relationships
The RPN is an integral part of the interdisciplinary team who collaborates with all team members and community partners about patient needs.
- Contributes to the overall functioning of the care team/unit/program
- Collaborates with team members to ensure continuity of patient care.
- Identifies and is involved in ethical discussion related to patient care
- Interacts with other units/programs/teams/departments to ensure quality patient care and appropriate resource utilization
- Participates in discharge planning for patients in collaboration with the patient/family and interdisciplinary team members
The RPN is involved in quality improvement initiatives, to meet the needs of our patients, families and groups.
- Identifies opportunities for improvement related to safe patient care and service delivery
Involved in continuous quality improvement initiatives
QUALIFICATIONS:
- Graduate of a diploma Registered Practical Nurse program from a Community College.
- A member in good standing with the College of Nurses of Ontario or eligible for and seeking registration with the College of Nurses of Ontario.
- Demonstrated knowledge of the CNO standards that apply to the RPN scope of practice in the area of hire.
- Excellent organization and communications skills.
- Sound problem solving, conflict resolution and decision making skills.
- Ability to function independently within scope of practice.
- Basic computer skills.
- Ability to cope with stressful work environment.
- Proven ability to attend work regularly.
- Satisfactory criminal reference check and vulnerable sector search required.
PHYSICAL REQUIREMENTS:
The applicant must be able to meet the physical demands of this position.
