Job Summary
Works with a multi-disciplinary health team to provide primary care services to individuals and families residing in the PQCHC area using a holistic model that emphasizes prevention, health maintenance, and education. The Nurse Practitioner functions as the Primary Care Provider, working autonomously to full scope managing their own panel of clients in accordance with the College of Nurses of Ontario (CNO) standards.
Job Specific Responsibilities:
- The NP works independently, serving as the Most Responsible Provider for a dedicated panel of clients, with access to physician consultation as appropriate.
- Diagnose, treat, and manage acute and chronic conditions across the lifespan.
- Collaborate with physicians, registered nurses, allied health professionals, and social services staff to deliver team-based primary care.
- Conduct comprehensive health assessments, develop and review care plans, and provide direct clinical services.
- Order and interpret diagnostic investigations, prescribe medications, and coordinate referrals to specialists and community resources.
- Educate clients on disease prevention, health promotion, and chronic disease management.
- Maintain accurate and timely documentation in the electronic medical record in accordance with professional and organizational standards.
- Participate in team meetings, case conferences, quality improvement, and continuing professional development activities.
- Support clients experiencing health inequities linked to the social determinants of health, such as income, housing, and employment.
- Participating in the Centre's efforts to enhance its capacity through staff development.
- Working in a manner that supports the Centre's Occupational Health and Safety policies and procedures.
Qualifications:
- Master's degree in Nursing and current registration in good standing with the College of Nurses of Ontario (CNO) in the Extended Class (NP-PHC or NP-Adult).
- Minimum of two (2) years of experience practicing as a Nurse Practitioner, ideally in a primary care or community health setting.
- Demonstrated experience or strong interest in working with one or more of PQCHC's priority populations: newcomers and refugees, older adults, 2SLGBTQ+ and gender-diverse clients, and people experiencing health inequities linked to the social determinants of health, such as income, housing, and employment.
- Experience in a community setting desirable.
- Demonstrated commitment to and knowledge of community-based health care.
- Technology proficiency an asset – PS Suite EMR, Microsoft Office Suite.
- Flexible hours, including evenings.
- Vehicle and license requirements.
- Excellent communication and interpersonal skills.
- Linguistic profile: Bilingual English/French is an asset.
- Other languages an asset.
