Nisga’a Valley Health Authority (NVHA) is seeking an experienced Registered Nurse (RN) or Licensed Practical Nurse (LPN) to join our growing Home & Community Care Team serving the four Nisga’a communities of the Nass Valley in beautiful northwest British Columbia.
Rather than working within the walls of a hospital, you will build meaningful relationships with Elders, clients, and families in their homes and communities. You will play a key role in helping individuals maintain their independence, coordinating care across the healthcare system, supporting hospital discharges, and providing clinical leadership within a rapidly growing community-based program.
The successful candidate will provide direct nursing services, conduct assessments, coordinate care plans, facilitate family meetings, mentor Health Care Aides, and collaborate with physicians, pharmacists, hospitals, specialists, and community partners to ensure safe, high-quality, client-centered care.
Key Responsibilities
Conduct nursing assessments and develop care plans
Coordinate hospital discharges and transitions home
Facilitate family meetings and care planning discussions
Provide wound care, chronic disease management, medications support, and palliative care services
Collaborate with physicians, pharmacists, hospitals, Northern Health, and community partners
Provide clinical leadership and support to Health Care Aides
Maintain accurate clinical documentation and electronic records
Travel throughout the four Nisga’a communities providing home-based nursing services
Qualifications
Current BCCNM registration in good standing as an RN or LPN
Valid Class 5 Drivers License
Current CPR certification
Minimum two years nursing experience preferred
Experience in Home and Community Care, Primary Care, Rural Nursing, Indigenous Health, Chronic Disease Management, Long-Term Care, Gerontology, or Palliative Care consistent an asset
Strong assessment, communication, organizational, and problem-solving skills
Demonstrated ability to work independently and make sound clinical decisions
Experience coordinating care with interdisciplinary healthcare teams
Experience providing leadership, mentorship, or supervision considered an asset
Satisfactory Criminal Record Check, including vulnerable sector screening
Knowledge of Indigenous health, cultures, traditions, and community-based healthcare practices is considered a strong asset
Pay: $62.00-$76.00 per hour
