DUTIES:
- Uses nephrology nursing standards of practice to prepare for patient clinic visit, complete patient assessment during visit and provide appropriate follow-up care, post visit, in collaboration with multidisciplinary team.
- Provides, as member of multidisciplinary team, group/individual patient/family education related to all stages of renal disease to optimize renal function.
- Supports Modality Coordinator in providing Kidney Care Clinic patients and family addition education regarding treatment options
- Provides quality education, monitoring, telephone and in-home support, as indicated, for independent dialysis (PD) patients. Collaborates with external service providers regarding PD services, supply deliveries and inventory.
- Performs peritoneal exchanges to inpatients receiving peritoneal dialysis.
- Supports pre-transplant work-up for eligible patients and post-transplant management in collaboration with multidisciplinary team and appropriate transplant centre.
- Understands and applies all infection control practices and precautions as required by practice setting
- Understands and complies with rules and regulations, requirements, and procedures of Health & Safety Program.
- Practices according to the Standards from College of Nurses, Ontario Renal Network(ORN) guidelines and targets, Canadian Association of Nephrology Nurses and Technicians (CANNT) guidelines and Clinical Practice guidelines of National Kidney Foundation Dialysis Outcomes Quality Initiative (K-DOQI).
- Practices independently and collaboratively with other health care providers in provision of individualized patient care for all renal populations.
- Effectively uses Standards of Nursing Practice and nursing process in assessing, planning, implementing, and evaluating care of pre-dialysis, home dialysis, hemodialysis and community transplant populations.
- Compliant with legislation, and organizational and departmental policies, standards, and procedures.
- Maintain commitment to self-growth and professional development.
- Demonstrate good interpersonal and conflict resolution skills.
- Remain current in knowledge and application of best practice through utilization of appropriate research and literature.
- Participates in continuous quality improvement opportunities to improve quality of nephrology patient care and patient satisfaction.
- Participate in research opportunities.
- Other duties as assigned.
QUALIFICATIONS:
- Graduate of an accredited Practical Nursing program.
- Certificate of Competency in Ontario.
- Basic Cardiac Life Support (BCLS) competency,
- Current certification in venipuncture (I.V. certification).
EXPERIENCE:
- Two years of recent acute medical experience required.
- Demonstrated clinical competency in care of Nephrology patients.
SILLS & ABILITY:
- Ability to perform peritoneal dialysis.
- Ability to work effectively and efficiently as a team member in a fast paced environment in stressful situations.
- Computer literacy in a Microsoft environment.
- Ability to use Meditch applications
- Demonstrated ability to maintain successful working relationships with the interdisciplinary team to achieve positive patient outcomes.
- Ability to organize time effectively to perform the duties of the position.
- Ability to meet and interact with people in a pleasant, professional, responsible and reassuring manner.

- Ability to read, write and communicate to perform duties of the position.
