Our new family practice is seeking a motivated and compassionate Nurse Practitioner to join our team. We pride ourselves on delivering comprehensive and patient-centered care, and we are committed to providing a supportive and collaborative work environment. Responsibilities include:
- Conduct comprehensive health assessments, diagnose and treat common acute and chronic conditions within the scope of practice.
- Provide timely episodic care for acute issues, ensuring efficient assessment, management, and appropriate follow-up.
- Order, interpret, and evaluate diagnostic tests and prescribe appropriate medications.
- Develop and implement individualized care plans in collaboration with patients and their families.
- Provide patient education and counseling on disease prevention, wellness promotion, and health maintenance.
- Collaborate effectively with our team of Nurse Practitioners and Family Doctors to ensure optimal patient care and continuity of services.
- Maintain accurate and up-to-date medical records using our electronic health record system.
Qualifications:
- Current and valid Nurse Practitioner license in Ontario.
- Demonstrated clinical competence in providing primary care services to patients across the lifespan.
- Strong interpersonal and communication skills, with the ability to build rapport with patients and collaborate effectively within a multidisciplinary team.
- Proficiency in using electronic health record systems.
