Position Summary
We are seeking a Program Coordinator to help plan, deliver, and continuously improve our interprofessional programs and services.
This is a leadership role with staff reporting to you; you will support a high-performing team while partnering with allied health professionals, physicians, and leadership to coordinate program operations, lead quality improvement and performance reporting, and strengthen partnerships that support high-quality, patient-centered care. This position reports to the Executive Director.
Responsibilities
- Coordinate planning, implementation, and evaluation of clinical and non-clinical programs in collaboration with the Executive Director and interdisciplinary teams
- Lead quality improvement activities, including development, tracking, and annual reporting of the Quality Improvement Plan (QIP)
- Lead performance and program reporting (e.g., Ontario Health and other required submissions), including data collection, analysis, and dashboarding
- Facilitate program and team meetings (e.g., quarterly program meetings; regular team meetings) and support follow-up on action items
- Build and maintain relationships with community partners to support coordinated, integrated services
- Provide day-to-day leadership for direct reports, including onboarding, coaching, and supporting performance development
- Support operational needs for allied health services (documentation, workflows, resources, and training, as needed)
- Support EMR/technology-enabled work (data pulls, documentation standards, and basic technology coordination)
- Coordinate communications and promotion of Rideau Family Health Team programs (website, newsletters, and social media) in collaboration with staff
- Other duties and projects, as assigned
Qualifications
- Undergraduate degree in health related or quality discipline (graduate education preferred)
- Minimum of 5 years progressive experience in program coordination, health services administration, quality improvement, or project management (health care setting preferred)
- Experience providing people leadership (e.g., supervision, coaching, onboarding, and supporting performance development)
- Knowledge of Ontario’s primary care landscape; experience in a Family Health Team (FHT), Community Health Centre (CHC), or other interprofessional primary care environment is an asset
- Demonstrated ability to design, implement, and evaluate programs; comfortable working with performance indicators and reporting requirements
- Strong analytical, organizational, and stakeholder engagement skills; able to manage multiple priorities
- Proficiency with EMR systems, data management, and Microsoft Office
- Understanding of privacy and relevant legislation (e.g., PHIPA) and comfort working in a regulated environment
- Commitment to equity, diversity, inclusion, and culturally safe care
- Valid driver’s license and access to reliable transportation
Working Conditions
- Full-time permanent (37.5 hours/week), Monday to Friday
- Office located in Ottawa, ON with free on-site parking
- Standard office environment within a primary care clinical setting
- Hybrid work arrangement (minimum 3 days/week in office)
- Occasional local travel to affiliated/satellite sites or community events
- Some evening or weekend hours may be required for programs or meetings

Pay: $75,000.00-$78,000.00 per year
