Education: College, CEGEP or other non-university certificate or diploma from a program of 1 year to 2 years or equivalent experience
· The Diversion Support Worker is a specialized Housing Support Role within the multi-facetted Community Development team. The Diversion Support Worker is responsible for supporting individuals exiting institutions, typically health care or corrections, with acquiring and/or stabilizing safe housing.
· The successful candidate will be responsible for co-creating case plans with clients, advocating for clients with housing providers, developing sustainability plans on a case-by case basis, system navigation, and documentation. This position is based in Amherst, however travel throughout Cumberland County is required.
· An understanding of harm-reduction practices and housing-first approaches are an asset.
· Additional responsibilities include develop and implement community support and development initiatives to further the YMCA’s mission and vision. Build and foster community partnerships in the delivery of these initiatives which may include leading or participating on committees relevant to this position. Participating in coordinated access systems to ensure community collaboration. Support other Community Development team.
· Other duties as required.
