Physiotherapist (1.0 FTE – 37.5 hrs./week)

We are looking for a full time Physiotherapist to join our interdisciplinary health care team. This position will work from both the Harrow Health Centre: Family Health Team as well as the Amherstburg Family Heath Team.

All the responsibilities we’ll trust you with:
· Responsible for the delivery of client-centered physiotherapy services
· Provides physiotherapy assessment, goal-oriented treatment interventions, client/family education and support to address medical and emotional needs
· Performs as a coach/mentor, consultant and uses sound critical thinking skills to achieve client outcomes and facilitate team functions
· Participates in program planning, continuous quality improvement and evaluation
· Review client clinical information obtained through the physician or IHP referral, laboratory results and health records
· Conduct consultations with clients/family to obtain relevant information and assess client/family learning and needs
· In collaboration with the Family Physician and other members of the interdisciplinary team, develop and discuss appropriate individualized care plan with the client/family/caregiver based on best practice
· Provide ongoing case management for patients
· Document all health notes into the EMR daily and arranged necessary follow up if required
· Prepare quarterly reports and monitor daily statistical data to ensure statistics are reported to the Ministry of Health in a timely fashion
· Actively participate in staff, team and committee meetings, if appropriate
All the responsibilities we’ll trust you with:
· Applicant must have access to a reliable vehicle.
· Must be licensed and in good standing with the Ontario College of Physiotherapists, expected to adhere to the Standards of Practice for Physiotherapists with the Ontario College of Physiotherapists
· Membership in the Canadian Physiotherapy Association preferred.
· Demonstrated experience in a Primary Care setting would be an asset
· Knowledge and proficiency in current Physiotherapy modalities and work within the areas of Controlled Act for which you are qualified
· Exceptional understanding of current, evidence-based and best practice physiotherapy guidelines
· Participate in individual and group education sessions for patients and providers
· Experience working in an electronic medical records environment; Experience with Accuro EMR is an asset
· Multi-tasking skills related to the delivery of efficient primary care including the ability to share information and teach while treating a patient
· Demonstrated ability to work effectively in a multi-disciplinary environment.
· Demonstrated commitment to and knowledge of community- based health care.
· Awareness of and sensitivity to the health issues of a diverse community
· Excellent communication, interpersonal, organizational and administrative skills including computer literacy
This position is a permanent 1.0 FTE position (37.5 hrs./week), with 2 job locations (60% Harrow Health Centre and 40% Amherstburg Family Health Team).
Example schedule:
Days: 8:30am - 4pm; 9:30am – 5pm
Evenings: 12:30 – 8pm
$39.07 - $43.07/hour (based on experience)
37.5 hours/week (5 days @ 7.5 hours)
