Job Overview
We are seeking a dedicated Registered Practical Nurse to join our team as a Clinical Supervisor. This position is being offered as a temporary casual position, with the potential to become a permanent position. The ideal candidate will help oversee our Personal Support Worker program, and possess strong leadership skills, clinical expertise, and a passion for providing exceptional community care. This role involves supervising Personal Support Worker staff, coordinating patient services, completing nursing assessments as part of new client intake processes, and ensuring compliance with healthcare standards. Experience in home care, assisted living, or nursing home settings is highly valued. The Clinical Supervisor will play a vital role in maintaining high-quality care delivery across various departments, including acute care, dementia care, and senior care.
Responsibilities
- Supervise and coordinate the daily activities of P.S.W staff to ensure efficient and effective patient care delivery.
- Complete nursing assessments as part of the client intake process, develop care plans with the client and family, complete all intake forms according to agency policies.
- Manage case coordination for diverse patient populations, including pediatrics, seniors, and individuals with dementia or chronic conditions.
- Ensure compliance with healthcare regulations and agency policies while maintaining high standards of safety and quality.
- Provide mentorship and training to P.S.W staff on delegated tasks, documentation protocols, and clinical best practices.
- Collaborate with interdisciplinary teams to develop individualized care plans and improve patient outcomes.
- Monitor clinical documentation for accuracy and completeness; ensure proper use of medical terminology in all records.
- Participate in quality assurance activities and contribute to continuous improvement initiatives within the community.
Requirements
- MUST be in good standing with the CNO and possess a valid license to practice.
- MUST have valid Professional Liability Insurance.
- MUST possess a Valid G or G2 driver's license
- MUST have access to own transportation and valid vehicle insurance.
- Proven experience in home care, assisted living, or nursing homes is preferred.
- Demonstrated team management and supervising experience within a healthcare environment.
- Strong knowledge of delegated procedures, dementia care, and case management.
- Familiarity with electronic health records systems such as AlayaCare.
- Competence in medical terminology, assessment skills, time management, and ability utilize resources effectively
- Excellent communication skills to effectively lead teams and collaborate with multidisciplinary staff.
- Ability to handle complex cases involving individuals across the healthcare spectrum.
