Summary:
Reporting to the designated Director of Professional Practice & Nursing, the Practice Consultant is responsible for provision of expert practice consultation and leading and coordinating professional practice and person and family-centred activities to ensure project deliverables are completed on time and on budget. The Practice Consultant:

Works with partners at all levels and inspires others to achieve goals and deliverables through facilitation, effective communication and addressing potential barriers to success.

Supervises staff and communicates with all levels of staff and management to facilitate consensus, consult, negotiate and share information to promote and advance interprofessional practice across PHC.

Liaises with staff, consultants, other health care entities and other external agencies, as needed.

Develops sustainable education plans and evaluates outcomes and acts as a resource for staff.
What You Bring:
Education:

Master's Degree in Nursing or other related health care discipline.
-
Seven (7) to ten (10) year's recent, related experience that includes managing large-scale projects, facilitating and managing consultation processes with a wide range of partner groups, or an equivalent combination of education, training and experience.

Current registration with appropriate licensing body.
Skills and Abilities:

Comprehensive knowledge of professional practice and person and family-centred care and application of standards to acute, long-term care, and community environments.

Comprehensive knowledge of project management principles and methodologies and ability to coach staff on these skills.

Comprehensive knowledge of appropriate professional practice or nursing regulations and their impact on clinical practice with a view to maximizing resource capacity.

Ability to utilize initiative, vision, independent thinking and creative problem-solving abilities to implement project plans and realize project completion.

Broad knowledge of patient care delivery systems in primary, acute and long-term settings.

Effective communication skills to function within a complex interdisciplinary environment involving physician and nursing communities.

Demonstrated ability to supervise staff.

Effective facilitation, persuasion and negotiation skills to achieve consensus, resolve conflict and achieve desired outcomes.

Ability to work effectively under time pressure to meet deadlines, balance work priorities and resolve problems in a timely manner.

Functional knowledge of word-processing, spreadsheet, presentation, project management and database applications.

A competitive salary: $108,147 - $155,461 per annum, depending on experience.

Your Day to Day:

Provides consultation and recommendations on professional practice and person and family-centred care, and on educational issues across PHC.

Collaborates with Practice Leaders, Program and/or Service Directors and Operational Leaders in addressing professional practice. Facilitates the promotion and advancement of interprofessional practice across PHC.

Supervises staff, determines performance expectations, provides performance coaching and evaluation for all direct reports. Establishes clear definitions of responsibility for each team member, reviews workload assignments and adjusts resources according to workload.

Develops detailed project charter and project plans outlining goals, objectives, timelines and project deliverables.

Tracks project progress according to project plan, monitors and reports on the status of projects and major issues/obstacles encountered. Makes recommendations regarding projects scope changes, rationale for change, seeks consensus and proceeds as needed. Acts as a resource to project team and staff, provides support and maintains project timelines.

Implements and/or contributes to the implementation of effective processes to assess project risks, identify risk mitigation strategies and monitor risk throughout the life cycle of the project.

In collaboration with Project Sponsor, develops a budget plan for the project that meets the goals and objectives of the project, monitors expenditures, identifies financial issues, and seeks to adhere to budget or take corrective action.

Communicates with partners, project sponsors and the organization on the scope and status of the project, benefits to patients and staff, obligations and responsibilities and changes in processes. Ensures questions and concerns are addressed in order to achieve buy in from staff.

Establishes project team and structure by gathering input from partners, identifying needs and skills required, establishing roles and responsibilities of team members, discussing timelines and explaining perimeters of project. Seeks input from the interdisciplinary team and recommends and/or negotiates contracts for provision of goods and services.

Develops and promotes flexible and sustainable education and training strategies and works with project team to gain commitment to implemented processes.

Develops a process of evaluation for project outcomes, data collection and analysis.

Participates on a variety of committees and establishes positive working relationships in order to ensure successful outcomes and cooperation from others on the project.
