Seeking a full-time LPN to provide medication administration, patient engagement, and OD response to people living in supported housing in Vancouver's Downtown Eastside, many with mental health and substance use challenges.
Qualifications:
- Member in good standing and current practicing registrant with the British Columbia College of Nurses and Midwives (BCCNM)
- Experience or interest in working with mental health and/or addictions is an asset
- Empathetic and comfortable working within a harm reduction framework
- Good communication skills
- Ability to work efficiently both independently as well as in a team
Job Responsibilities: ·
- Deliver direct nursing care as required
- Work efficiently with a focus on Medication Administration and Patient Engagement
- Using a structured process in collaboration with the client, assess, plan, deliver, evaluate, and document individualized client assessments that reflect the client’s goals and priorities. Provide consistent follow-up and change plans of care as needed
- Coordinate nursing care services with the client, physician, hospital, rehabilitation program, long-term care program, and other community services/programs. i.e., mental health, social development, nutrition programs, etc.
- Maintain confidential reporting and recording data systems
- Collaborate with interdisciplinary team members to ensure effective, consistent, and comprehensive care planning and delivery
