Overview
Heart2Hand Care is a GTA-based care coordination service. We connect clients with pre-screened, independent contractor PSWs. We handle the matching, scheduling, and billing so that you can focus on providing quality care. This is a non-exclusive, flexible contractor role. You are free to work with other agencies while taking assignments from us.
Duties
- Assist clients with activities of daily living including bathing, grooming, dressing, and toileting
- Support client mobility and transfers
- Provide companionship
- Provide medication reminders
- Prepare meals and assist with feeding as needed
- Perform light housekeeping and errand-running related to client care (e.g. dishes, laundry, cleaning)
- Document client care activities according to care plans
- Communicate with the coordination team as needed
Requirements
- Prior experience in assisted living, nursing home, hospice, or home care settings preferred
- Knowledge of patient care, senior care, dementia care, and working with individuals with disabilities
- Ability to perform heavy lifting safely and assist with mobility support
- Experience with medication administration and meal preparation is desirable
- Compassionate attitude with excellent communication skills
- Ability to work independently and follow detailed care plans
- Relevant certifications or training in caregiving or nursing assistance is a plus
- Flexibility to adapt to different client needs and work environments
- Valid PSW Certificate
- Standard First Aid & CPR-C
- Vulnerable Sector Police Check (renewed every 2 years)
- Up-to-date immunizations including COVID-19, Measles, Mumps, Rubella, and Tetanus
- $2 million liability insurance (naming Heart2Hand Care as additional insured)
- IPAC (Infection Prevention & Control) training (renewed annually)
- Reliable transportation to travel within the GTA
- Comfortable with independent contractor status (responsible for your own taxes, CPP and other benefits)
