Job Summary:
We are seeking a proactive and compassionate Nurse Practitioner (NP) with an interest in preventative health, and lifestyle medicine. The ideal candidate is passionate about empowering patients through health education, early intervention, and sustainable lifestyle changes. This is a unique opportunity to make an impact, while innovating within a supportive and forward-thinking environment.
Key Responsibilities:
· Be responsible and accountable for the comprehensive assessment of patients and clients, including the diagnosis of diseases, disorders, and conditions, with a strong emphasis on preventative care, early intervention, and health promotion.
· Independently perform comprehensive clinical procedures and assessments including, but not limited to, pap tests, skin assessments, immunizations, bloodwork, and comprehensive physical examinations with a focus on preventative care, early detection and chronic disease management
· Maintain accurate, detailed, and timely patient records in compliance with legal and organizational standards.
· Ensure compliance with regulatory, ethical, and organizational standards in all program-related activities
· Possibility of working additional hours, as well as educational seminars and events, outside of typical work day hours
Qualifications:
-Current registration as a Nurse Practitioner in good standing with the College of Nurses of Ontario (CNO).
-Current professional liability protection and eligibility to practice in Ontario.
-Proven experience as a Nurse Practitioner, with clinical experience in primary care, urgent care, community health, or a relevant specialty.
-Strong clinical assessment, diagnostic, critical thinking, and decision-making skills.
-Demonstrated ability to develop, implement, and evaluate comprehensive treatment and care plans.
-Interest in or experience with preventative health, lifestyle medicine, health optimization, and chronic disease prevention.
-Knowledge of evidence-based approaches to metabolic health, cardiovascular health, weight management, hormone health, and healthy aging is considered an asset.
-Commitment to patient education, health promotion, and empowering patients to take an active role in their health.
-Ability to work independently while collaborating effectively within an interdisciplinary team.
-Exceptional communication skills, both verbal and written.
-Strong organizational, time management, and documentation skills with attention to detail.
-Familiarity with electronic medical records (EMR/EHR) systems and virtual care platforms.
-Ability to build strong therapeutic relationships and deliver care with empathy, professionalism, and cultural sensitivity.
-Demonstrated commitment to ongoing professional development and evidence-informed practice.
-Experience with quality improvement initiatives, program development, or clinical leadership is considered an asset.
-Certification or additional training in lifestyle medicine, obesity management, hormone therapy, women's health, men's health, functional health assessment, or preventative medicine is considered an asset.
