About The Role:
Moira Place Long Term Care Home is seeking an Attending Physician to support our dedicated care team. In this role, you will provide comprehensive medical care to residents while working both independently and collaboratively within an interdisciplinary team.
As a key clinical partner, you will support high-quality, resident-centred medical care, contribute to continuity of care, and ensure medical services align with each resident’s needs, goals, and wishes. Key Responsibilities
· Provide comprehensive medical assessments, diagnosis, treatment, and ongoing evaluation of residents’ health status.
· Develop and implement individualized medical plans of care in collaboration with the interdisciplinary team.
· Participate in care planning discussions and maintain open communication with residents and their families/representatives.
· Complete admission, readmission, and annual medical assessments in a timely manner.
· Document all clinical findings, assessments, and care provided in the resident health record to ensure continuity of care.
· Provide ongoing medical oversight and support for acute and chronic conditions within the long-term care setting.
· Collaborate with Medical Director, nursing staff, and allied health professionals to support best practices in resident care.
· Work in alignment with the Home’s policies, standards, and protocols, while upholding professional and regulatory standards of care.
· Support and, where appropriate, participate in quality improvement initiatives to enhance clinical care delivery.
· Collaborate with other attending physician to provide or arrange for 24-hour on-call medical coverage for residents under your care.
· Communicate appropriate alternatives for emergency medical services when unavailable.
Qualifications
· Legally qualified medical practitioner licensed to practice medicine in Ontario.
· Current and valid malpractice protection, maintained in good standing.
· Experience in long-term care, geriatrics, primary care, or community-based practice is preferred.
· Strong clinical judgment, leadership, and ability to work autonomously and collaboratively.
· Strong knowledge of geriatric syndromes, dementia care, and chronic disease management.
· Proficiency with EMR systems and Microsoft Office.
