Overview
We are seeking a highly skilled and compassionate Advanced Nurse Practitioner to join our healthcare team. This role offers an opportunity to provide comprehensive patient care across diverse settings, including pediatrics, senior care, acute care, and home health. The ideal candidate will have extensive experience working with children, seniors, and in various clinical environments such as hospitals, nursing homes, and assisted living facilities. The Advanced Nurse Practitioner will play a vital role in delivering exceptional patient-centered care, managing complex cases, and collaborating with multidisciplinary teams to improve health outcomes.
Duties
- Conduct thorough patient assessments and develop individualized care plans for diverse populations including pediatrics and seniors.
- Perform comprehensive patient care activities such as medication administration, pain management, and wound care.
- Utilize electronic health records systems such as Epic and Cerner for documentation and case management.
- Manage acute and chronic conditions, including dementia care and other complex medical issues.
- Coordinate with healthcare teams to ensure continuity of care across settings like hospitals, nursing homes, assisted living facilities, and home care environments.
- Assist with laboratory procedures including phlebotomy and interpret laboratory results to inform treatment decisions.
- Provide patient education on disease management, medication use, and preventive health strategies.
- Collaborate with physicians and other healthcare professionals to optimize treatment plans and improve patient outcomes.
- Maintain up-to-date knowledge of medical terminology, clinical protocols, and emerging healthcare practices.
Qualifications
- Valid Advanced Nurse Practitioner license with current certification.
- Experience working with children, seniors, or in acute hospital settings is highly preferred.
- Background in nursing home or assisted living environments is advantageous.
- Proficiency in medical terminology, medication administration, and laboratory procedures such as phlebotomy.
- Familiarity with electronic health record systems like Epic and Cerner.
- Strong case management skills with the ability to coordinate complex patient care plans effectively.
- Excellent patient care skills with a compassionate approach tailored to diverse populations including pediatrics and dementia patients.
- Prior experience in pain management, home care, or senior care is desirable.
- Ability to work collaboratively within multidisciplinary teams in fast-paced clinical settings.

Pay: $30.00-$50.00 per hour
