Role: The Guelph FHT is looking for a Full-Time Contract (1.0 FTE) Diabetes Educator (CDE), Registered Dietitian for our Diabetes Care Guelph Team
Role Description: Position Summary
The Diabetes Educator, Registered Dietitian plays a key role within the Diabetes Care Guelph (DCG) education program—working collaboratively with multidisciplinary team that includes dietitians, nurses, kinesiologists, and allied health professionals.
As an integral member of this patient-focused team, the dietitian delivers comprehensive diabetes education and individualized counseling for adults living with diabetes, both one-on-one and in group settings. The role includes supporting clients with Gestational Diabetes Mellitus (GDM) and Type 1 Diabetes.
This position involves providing education and support both on-site and through occasional community outreach, contributing to improved self-management, empowerment, and overall health outcomes for individuals living with diabetes.
Position Responsibilities:
- Provide individualized diabetes education to individuals with diabetes
- Manage a large dynamic caseload, including initial assessments and follow-up visits.
- Prepare, plan, deliver, and document monthly education as needed
- Deliver education on diabetes-related topics such as disease process, medication, diet, nutrition, foot care, healthy lifestyle, prevention and management of complications, blood glucose monitoring, injection and the techniques of self-monitoring
- Review blood glucose readings for the purpose of medication control and adjustment
- Conduct ongoing assessment and provide recommendations to patients' healthcare providers and caregivers for adjustments in care plan as required
- Maintain thorough documentation including assessments, interventions, progress notes and discharge summaries
- Collaborate with other DCG colleagues (RNs and RKins) to develop/implement chronic disease management programs based on population needs.
- Foster a collaborative care approach within the Family Health Team through effective communication and mutual respect.
- Provide internal workshops or seminars to support the professional development of team members in diabetes management as needed. Communicate relevant nutritional information to other health care providers in the community
- One evening shift a month is required as part of this role (11am to 7pm) as well as teaching or co-teaching group classes as needed
Qualifications:
- Current registration with the College of Dietitians of Ontario, in good standing
- Certification as a Certified Diabetes Educator (CDE)
- Experience counselling patients with diabetes, incorporating behaviour change models, adult learning principles and techniques for chronic disease management
- Experience with insulin therapy; initiation and adjustment
- Experience in providing education to adults with Pre-diabetes, Type 1 and Type 2 diabetes and gestational diabetes (GDM) is considered an asset
- Comprehensive competency in diabetes management is required
- Familiarity of insulin pump therapy; certification in pump training considered an asset.
- Ability to work autonomously and as a member of an interdisciplinary team
- Critical thinking skills
- Comfortable and experienced with individual and group presentations, in person and virtually
- Knowledge of and sensitivity to the impact of social, economic, environmental, and cultural issues in diabetes self-management
- Familiarity with Telus Practice Solutions is considered a strong asset
- Craving Change (TM) Certification is an asset
