Cedars' Continuing Care program provides one year of structured post-treatment support, including one-on-one recovery coaching, facilitated recovery groups, alumni programming, family services, and supportive recovery resources designed to strengthen long-term recovery.
POSITION SUMMARY :
Reporting to the Manager of Continuing Care, the Continuing Care Coordinator provides administrative, operational, and program coordination support across Cedars’ continuing care, outpatient, family, alumni, and supportive recovery services. The Coordinator supports individuals as they transition from residential treatment, coordinates program operations, maintains accurate records and reporting systems, and ensures effective communication between participants, families, staff, referral and funding partners, community agencies, and internal departments.
The successful candidate is highly organized, collaborative, and relationship-focused, with excellent communication skills, sound judgment, and a commitment to maintaining professional boundaries, confidentiality, and a recovery-oriented approach.
DUTIES AND RESPONSIBILITIES:
The duties listed below are representative of the role and are not exhaustive. The Continuing Care Coordinator may be required to perform other related duties consistent with the position and the operational needs of Cedars Recovery.
Program and Operational Coordination
- Coordinate the day-to-day operational flow of continuing care, which includes alumni, family members, community members, and individuals transitioning from treatment.
- Maintain program schedules, participant lists, attendance records, trackers, and other departmental data within Cedars EHR and externally as required.
- Support appropriate program placement and enrolment of patients, alumni, family and community members in continuing care programming.
- Prepare weekly updates for the Manager of Continuing Care regarding patient housing, identify any case management or scheduling challenges for the team, family program enrollment and other weekly departmental needs.
- Support the planning, implementation, evaluation, and improvement of existing and new programs.
- Prepare program materials, presentations, templates, forms, attendance tools, and other operational resources.
Patient, Family, and Stakeholder Support
- Meet with individuals transitioning from treatment to champion Continuing Care services: supportive recovery living, respond to routine questions or concerns, and the recovery continuum of care.
- Monitor participant engagement and proactively identify individuals who may benefit from additional outreach or support, escalating concerns to the appropriate team member when required.
- Provide timely and professional information regarding program enrolment, scheduling, payment, and participation requirements.
- Maintain clear communication between patients, families, alumni, referral partners, employers, community agencies, and internal departments.
- Support individuals in navigating available recovery resources by facilitating connections with referral sources, community partners, and other supports that promote continuity of care and successful recovery outcomes.
- Contribute to a welcoming, recovery-oriented environment that reflects Cedars' commitment to connection, belonging, and lifelong recovery.
- Represent the Continuing Care department professionally in interactions with patients, alumni, families, the public, and external agencies.
- Maintain and build collaborative relationships with community partners and other service providers.
- Support monthly Alumni Milestone meetings as well as Cedars annual events through planning and attendance.
Documentation, Reporting, and Administrative Support
- Maintain accurate, current, and confidential Continuing Care files and patient records in accordance with Cedars’ policies and documentation standards.
- Prepare and maintain case conference minutes, meeting notes, reports, correspondence, and departmental records as directed.
- Track participant registration and provide accurate information to Accounting regarding billing and payment processes.
- Monitor program participation, service activity, and departmental data, and assist with outcome reporting and quality improvement.
- Provide administrative support, special project assistance, and other assignments delegated by the Manager of Continuing Care.
Team Collaboration and Professional Practice
- Collaborate with Continuing Care staff and other departments to support continuity of care and coordinated service delivery.
- Communicate concerns, scheduling issues, participant needs, and operational barriers promptly to the appropriate leader.
- Maintain professional boundaries, confidentiality, and respectful communication in all interactions.
- Model a calm, organized, solution-focused, and recovery-oriented approach.
- Maintain current knowledge of Cedars’ policies, procedures, programs, and communication expectations.
QUALIFICATIONS:
Required
- Two or more years of experience in program coordination, administration, client service, health care, human services, or a related environment.
- Strong organizational, scheduling, record-keeping, and follow-through skills.
- Exceptional interpersonal skills with the ability to build trust and foster collaborative relationships.
- Excellent verbal and written communication skills.
- Demonstrated ability to manage multiple priorities, deadlines, and changing operational needs.
- Strong computer skills, including Microsoft 365 and the ability to learn electronic records and tracking systems.
- Ability to maintain confidentiality, professional boundaries, and sound judgment when handling sensitive information.
- Ability to work effectively with patients, families, staff, referral partners, and community agencies.
- Satisfactory Criminal Record Check, including the Vulnerable Sector.
Preferred
- Post-secondary education in business administration, health administration, social services, counselling, psychology, addictions, or a related field.
- Experience in addiction treatment, mental health, continuing care, outpatient services, supportive recovery, or residential care.
- Experience with program enrolment, participant tracking, billing coordination, meeting documentation, or outcome reporting.
- Experience using electronic health records (EHRs) and maintaining documentation in accordance with clinical and organizational standards.
- Knowledge of recovery-oriented, trauma-informed, and culturally safe approaches to service delivery.
- Valid Class 5 BC driver’s license and clean driver’s abstract.

WORKING CONDITIONS:
- This is a full-time, primarily on-site position based in Cobble Hill, BC.
- The regular schedule is Monday to Friday, with occasional flexibility required to support program delivery, meetings, events, or operational needs.
- The role involves regular computer and telephone work, interaction with patients and families, and coordination across multiple departments and external partners.
- Some local travel may be required for program, community, or partner-related activities.
