The Alzheimer Society Brant, Haldimand Norfolk, Hamilton Halton (ASBHNHH) is the local community agency dedicated to supporting people living with dementia, their care partners, and families. The Opportunity
Under the direction of the Team Lead, Counselling, this position provides counselling and support services to persons living with dementia (PLWD) and their care partners. These services are delivered in-office, home visits, and phone, using both individual and group approaches. This position is based in Georgetown with travel throughout Halton region. Office hours are Monday to Friday from 8:30 am to 4:30 pm, however occasional evening and weekend work may be required.
What You will Do
- Conducts relevant assessments to identify current and future needs, goals and risks
- Create and update care plans to address needs, goals and risks
- Provide supportive counselling to PLWD and/or their care partners
- Provide individualized education on topics related to dementia and caring for a person with dementia.
- Facilitate therapeutic support groups.
- Provide system navigation to connect clients with support and services based on assessed needs, including but not limited to ASBHNHH services, community agencies, the health care system, and government services.
- Enter, update, and maintain client notes in the organization’s database within the required documentation timeframe.
- Maintain confidential, accurate client records in compliance with privacy legislation, professional standards, and internal policies.
- Participate in care review meetings with other ASBHNHH services and/or external services to ensure successful client transition and service delivery as needed.
- Continually enhance knowledge of dementia, research and available resources by attending presentation and/or training as applicable.

What You Bring
We are looking for someone who combines compassion with sound judgement and strong communication skills:
- Degree or diploma in social work (MSW, BSW, SSW) or a comparable educational field
- Membership, in good standing, with a professional college
- Must have in-depth knowledge and understanding of Alzheimer’s disease and other dementias
- Demonstrated experience providing counselling services to older adults, PLWD and/or their care partners
- Demonstrated experience facilitating therapeutic support groups
- Knowledge of services, supports, clinical, social, and institutional care available in the community.
- Knowledge of consent and capacity issues and legislation related to high-risk elderly
- Understanding of the role and linkages between primary care, community care and specialized geriatric providers
- Ability to work independently and in a team environment.
- Ability to use discretion, sound judgment and tact in handling sensitive/confidential information and situations.
- Excellent communication, interpersonal, and presentation skills
- Proficiency with Microsoft Office Suite applications and databases
- Vulnerable sector police check issued within 3 months of the hiring date
- Must have regular access to a mobile phone capable of downloading the Microsoft Authenticator app to support two-factor authentication for IT security purposes
- Valid driver’s license and regular access to a reliable vehicle
- Evening and/or weekend work may be required
- Ability to speak a second language is an asset.
