Job Summary
EmployAbilities is looking for a Case Manager supporting the EAN program. The successful candidate will provide trauma-informed employment support to individuals facing disabilities and barriers to employment, helping them build stability, develop goals, and move toward meaningful employment opportunities.
What You’ll Do
· Conduct assessments and develop personalized action plans that help participants move toward goals.
· Connect participants with community resources and wraparound supports such as housing, transportation, health, and employment services.
· Provide ongoing coaching, encouragement, and case management while collaborating with facilitators, peer support staff, and community partners.
What You’ll Bring
· Diploma or degree in human services, social work, psychology, career development, or a related field, or an equivalent combination of education and experience.
· Experience supporting individuals facing barriers to employment, housing, health, or other life challenges.
· Experience conducting assessments, developing action plans, or providing case management and client support services.
Needed Skills
· Trauma-informed and person-centred approach
· Strong assessment and problem-solving skills
· Excellent communication and relationship-building abilities.

Pay: $47,000.00-$51,000.00 per year
