We operate a community social pediatrics clinic, offer wraparound programs for children and youth, and collaborate with families and community partners to address the root causes of poor health and social outcomes.
The Role: Community Social Pediatrics School and Health Liaison
The Community Social Pediatrics School and Health Liaison will play a vital role in bridging the gap between education, healthcare, and social supports. This position will be embedded within two local Saint John schools, working directly with school staff, families, and NBSP's clinical team to support children experiencing barriers related to poverty, trauma, and limited access to care. School assignments may be adjusted within NBSP's service area based on program needs, funding, school partnerships and operational requirements. This is a unique opportunity to be part of a wonderful new initiative in New Brunswick, improving outcomes for children and shaping a model of care that could be replicated across the province.
Key Responsibilities:
- Build and maintain strong relationships with school staff, children, families, and community organizations.
- Work within local schools to develop trusting relationships between NBSP and partner school teachers, administration, and leadership teams.
- Identify children and families in need of health and social supports, especially those without a primary care provider.
- Facilitate access to NBSP services, including pediatric care, mental health support, advocacy, and community-based programs.
- Coordinate case conferences and participate in school team meetings focused on student well-being.
- Act as a health navigator to connect families with community, healthcare, and government services.
- Liaise with professional service providers (Occupational Therapy, Physical Therapy, Speech Language Pathology, Psychological support services, mental health resources, etc.)
- Support early screening and identification of developmental, behavioural, and emotional challenges.
- Maintain accurate, timely, and professional case notes and program records to support clinical and program evaluation using only Organization-approved systems, and comply with all consent, privacy, confidentiality, and information-sharing requirements.
- Champion trauma-informed, child-centered approaches in schools and support staff in understanding Adverse Childhood Experiences (ACEs).
- Collaborate with NBSP's clinical team (pediatrician, nurse, other liaisons) to ensure continuity of care and coordinated service delivery.
- Participate in program evaluation and research to support evidence-based practice and policy development.
Qualifications
- Degree or diploma in Social Work, Child and Youth Studies, Nursing, Education, or a related field. Equivalent experience may be considered.
- Minimum 3 years of experience working with children and families facing adversity.
- Demonstrated understanding of the social determinants of health and Adverse Childhood Experiences (ACEs).
- Strong interpersonal and communication skills with the ability to work collaboratively in a multidisciplinary environment.
- Knowledge of community resources and service navigation in the Saint John area.
- Experience in either the education sector (e.g., school settings or school-community partnerships) or the healthcare sector (e.g., clinical settings or health system navigation) is required.
- Confidence and skill in working with children experiencing emotional and behavioural challenges is required.
- Ability to manage sensitive information with discretion and professionalism.
- Ability to work across multiple digital platforms is required, including comfort using tools such as Outcome Tracker (Vistashare), Electronic Medical Records, email, shared drives, and virtual meeting platforms.
- Lived experience as a member of a marginalized community, including but not limited to experiences of poverty, trauma, systemic racism, or barriers to accessing care, is considered a strong asset.
- Clear Criminal Record and Vulnerable Sector Checks are required.
- Access to a valid driver's license and vehicle is required.
- Bilingualism (English/French) is an asset.
