We are seeking a dedicated Certified Diabetic Educator to join our healthcare team. The Certified Diabetic Educator will utilize electronic medical records and demonstrate excellent customer service skills to ensure a positive patient experience. Bilingual abilities and proficiency in computer operation are highly valued to effectively serve diverse patient populations.
Responsibilities
- Conduct individual and group education sessions on diabetes management, including medication adherence, nutrition, physical activity, and blood glucose monitoring
- Develop personalized care plans in collaboration with healthcare providers and patients
- Maintain accurate and up-to-date electronic medical records for all patient interactions and progress notes
- Provide ongoing support and counseling to motivate patients towards healthier lifestyle choices
- Assist patients in understanding their treatment options and answer questions related to diabetes care
- Collaborate with multidisciplinary teams to coordinate comprehensive patient care
- Utilize computer systems efficiently for documentation, scheduling, and communication purposes
- Offer bilingual support as needed to accommodate diverse patient populations
Experience
- Certification as a Diabetes Educator through recognized professional organizations is required
- Prior experience working in a clinical or healthcare setting is preferred
- Familiarity with electronic medical records systems is essential
- Strong customer service skills with the ability to communicate effectively and empathetically with patients
- Competence in computer operation and data entry tasks is necessary
