Position Overview
Modern Medical is seeking a Nurse Practitioner to work with a physician to provide primary care for first responders enrolled in the Guardian Program. The Guardian Program is a unique model that will integrate primary care, WSIB support and mental health services provided by an interdisciplinary team that work collaboratively.

Key Responsibilities
- Provide comprehensive primary care to patients enrolled within the Guardian Program
- Assess, diagnose, treat, and manage acute and chronic medical conditions
- Collaborate with physicians, allied health professionals, and operational leadership to coordinate patient care
- Facilitate referrals and coordination with specialists and community resources
- Maintain accurate and timely clinical documentation within the EMR system
- Participate in quality improvement initiatives and program development
- Contribute to a positive clinical environment
Qualifications
- RN license from College of Nurses of Ontario in good standing, designation required
- Completion of Nurse Practitioner (NP) Primary Care certificate is required
- Recent experience: Prefer previous working experience in a community-based or hospital based clinical team delivering related programs
- Current professional liability coverage
- Current Vulnerable Sector Check
- Proof of BCLS certification
- Strong communication and interpersonal skills
- Ability to work both independently and collaboratively within an interdisciplinary team
- Experience in primary care, chronic disease management, mental health, occupational health, or emergency medicine is considered an asset
- Competence and comfortability using technology, specifically in using electronic medical record systems.
