Overview
Nuvella Haven Homecare Staffing Canada is seeking a Registered Social Worker or Mental Health Therapist to support adults with developmental disabilities and complex service needs transitioning from hospitals into community-based environments.
Many individuals supported through this program may present with:
- Mental health diagnoses
- Trauma histories
- Addiction concerns
- Forensic involvement
- Complex behavioural presentations
- Family and social system challenges
This position plays a critical role in promoting stabilization, community integration, mental wellness, and successful long-term outcomes.
KEY RESPONSIBILITIES
Mental Health Support
- Conduct psychosocial assessments
- Support mental health stabilization
- Develop therapeutic interventions
- Coordinate treatment plans
Client & Family Support
- Provide counselling and emotional support
- Support families and guardians
- Facilitate conflict resolution and advocacy
Community & Clinical Coordination
- Coordinate referrals to community resources
- Collaborate with psychiatrists, healthcare providers, and support teams
- Participate in multidisciplinary case conferences
Risk Management
- Support crisis planning and intervention
- Assist with suicide risk assessments when required
- Develop strategies to prevent hospitalization and placement breakdowns
Documentation & Reporting
- Maintain clinical documentation
- Complete assessments and progress reports
- Participate in quality assurance reviews
QUALIFICATIONS
Required:
- Registration with ACSW or applicable regulatory body
- Degree in Social Work, Psychology, Counselling, or related field
- Minimum 3 years experience supporting vulnerable populations
- Strong knowledge of trauma-informed practice
- Valid licensure as a Licensed Clinical Social Worker (LCSW) or equivalent credential.
- Experience working with children and adolescents in clinical or hospital settings preferred.
- Knowledge of medical terminology, case management principles, and data collection methods relevant to mental health services.
- Familiarity with research methodologies related to pediatrics and mental health is a plus.
- Strong communication skills with the ability to work collaboratively within healthcare teams.
- Experience in special education environments or working with diverse populations is advantageous.
- Demonstrated commitment to patient-centered care and confidentiality standards.
