Position Summary
Milestone Community Care is seeking a dedicated and compassionate Outreach Support Worker to join our team. The successful candidate will provide direct support to adult clients in the community, assisting them with daily needs, appointments, advocacy, and community integration. This role requires strong interpersonal skills, reliability, and a genuine commitment to helping others.
Key Responsibilities
- Provide ongoing support and assistance to adults in our care.
- Conduct daily check-ins with clients to assess needs and provide support.
- Transport and accompany clients to medical appointments, court appearances, grocery shopping, and other community activities.
- Assist clients in accessing community resources and services.
- Support clients in achieving personal goals and maintaining independence.
- Advocate for clients and help navigate systems and services.
- Maintain accurate documentation and case notes.
- Build positive, professional relationships with clients, families, and community partners.
- Respond appropriately to client concerns and crisis situations when required.
- Ensure client confidentiality and uphold professional boundaries at all times.
Qualifications
- Diploma or degree in Social Service Work, Community Services, Human Services, or a related field is preferred.
- Experience supporting adults with diverse needs is an asset.
- Strong communication, problem-solving, and organizational skills.
- Ability to work independently and as part of a team.
- Valid Ontario Driver's License and reliable vehicle required.
- Clear Criminal Record Check and Vulnerable Sector Check.
- First Aid and CPR certification is an asset.
