Position Summary
The Home Visitor provides one‑to‑one outreach support to caregivers of children 0–6 years, including prenatal caregivers, in family homes across Fort Saskatchewan and the Sturgeon Region. This role is rooted in relationship‑building, meeting families where they are and strengthening the connections that help children thrive. Home Visitors walk alongside caregivers with gentle, trauma‑aware support, especially when families are navigating complex needs. They create a welcoming, non‑judgmental space where caregivers feel safe, respected, and empowered to share their stories and goals. Every visit is an opportunity to build trust, celebrate strengths, and support families in ways that honour their lived experiences.
The Home Visitor completes screenings and assessments to help caregivers understand their needs and collaborates with them to access community and partner supports when needed. This role also contributes to early childhood development programming at Families First Society and facilitates Health for Two services for prenatal participants, ensuring families feel connected to a caring community from pregnancy through early childhood.
Duties and Responsibilities
- Conduct developmental assessments using tools such as the Ages and Stages Questionnaire to understand caregiver and child needs and identify appropriate supports.
- Provide in‑home psycho‑educational support offering gentle, trauma‑aware guidance to caregivers and children in their homes, including those navigating complex needs.
- Support caregiver–child attachment by modeling healthy behaviours and providing education from trainings and approved theories of practice.
- Co‑create service plans and goals working with caregivers to build individualized plans that promote developmental growth, resilience, and family‑identified priorities.
- Engage caregivers and children in capacity building opportunities fostering confidence, skill‑building, and hope through strengths‑based activities and conversations.
- Coordinate community referrals supporting caregivers in navigating complex systems and connect families to partner agencies and community resources.
- Advocate for families ensuring caregivers can access the supports they require and receive equitable, respectful service across systems.
- Report trends/service gaps and share observations with the Families First team to strengthen programming and community responsiveness.
- Support data collection, evaluation, and reporting requirements.
- Build collaborative partnerships by fostering and maintaining strong relationships with community organizations across Fort Saskatchewan and the Sturgeon Region.
- Develop community knowledge by staying informed about local resources, emerging needs, and community strengths to better support families.
- Work effectively within a team and independently by collaborating with colleagues while also managing independent outreach and home‑based work.
- Participate in bi-weekly consultations with leadership to align work with FRN priorities, Home Visitation Guidelines, and best-practices.
- Participate in peer-support practices and cross-training initiatives set out by the agency
Qualifications
- Diploma or degree in human services field; or at least 4 years of work-related experience (Registration with your regulatory body if applicable)
- Knowledge of Fort Saskatchewan/Sturgeon Region communities, resources, and service systems.
- Experience supporting systems navigation, referrals, and community-based coordination.
- Skilled in screenings, assessments, documentation, case management, and service coordination.
- Excellent interpersonal skills/communication and strong relationship-based practice.
- Strong time management, organization, and ability to prioritize in a fast-paced, dynamic role.
- Solid understanding of child and adult development, brain science, trauma-informed care, attachment theory, and strength based approach.
- Experience working with individuals experiencing complex needs in high emotional states
- Experience with screening tools such as Ages and Stages Questionnaires, Family Violence Prevention Protocol, and suicide prevention frameworks.
- Ages and Stages Questionnaire training, Circle of Security Parenting considered an asset
- Flexibility to work days, evenings, and weekends as needed.
- Access to a reliable vehicle and a clean driving record.
- Ability to provide a criminal record check and intervention record check.
